This Licensed Practical Nurse will be working in the Inpatient Behavior Health Department, responsible for delivering designated nursing activities for patients, families and/or significant others experiencing acute and/or chronic medical problems, diagnostic, and/or surgical interventions, and/or psychiatric problems., located at the Harry S. Truman Memorial Hospital in Columbia, MO.
Duties
Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Ability to serve as a responsible member of the nursing team and interact in an appropriate manner with patients, family members, professional and other supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice.
Prepares and administers prescribed medications accurately and on time
Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD/DO.
Provides patient education about prescribed medications.
Recognizes when a medication error has been committed, reports immediately to team leader/charge nurse, and initiates incident report.
Holds current IV certification.
Monitors IV sites for signs of infiltration or infection and takes appropriate actions.
Monitors IV flow and rate.
Documents patient information in the record and at the bedside according to policy and procedure.
Documents in a legible, timely, accurate, and complete manner.
Seeks opportunities to participate on Nursing and hospital committees, and to be involved in hospital-wide activities. Reports committee activities.
Communicates in a professional, clear manner.
Documents communication of pertinent patient information to RN and physician.
Collaborates with RN on development of care plan. Contributes to the interdisciplinary team in the development, implementation, and evaluation of the patient's treatment plan

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
ENGLISH LANGUAGE PROFICIENCY: In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
EDUCATION: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. This requirement is waived for candidates with health care education or training in military service, which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure. This requirement is also waived for the conversion of VHS employees to appointment under authority of 38 U.S.C. 4104(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576 enacted October 28, 1986; and for their future personnel actions under this authority. This requirement is also waved if an LPN was licensed in a jurisdiction that does not require graduation from an approved school, provided the individual has demonstrated successful practice as an LPN/LVN. One year of successful practice is considered the equivalent of graduation from an approved school of 12 months duration.
LICENSURE: Full, active, and current licensure as a graduate licensed practical or vocational nurse in a State, Territory or Commonwealth of the United States, or District of Columbia. An LPN/LVN who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provision in chapter 3, section B, paragraph 16 of this part.
Grade Determinations: In addition to the basic requirements stated in section A, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
GS-3. None beyond the basic requirements. [LPNs at this grade level are expected to perform all duties within the scope of their license (e.g., medication administration); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved.]
GS-4. Six months of qualifying experience as an LPN or LVN; or Graduation from an approved school (which may be waived as provided in paragraph 4 of Section C) and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or Graduation from an approved school of at least 24 months duration.
GS-05. Completion of at least one (1) year of qualifying experience at the GS-4 level or equivalent. Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems.
GS-06. Completion of at least one (1) year of (additional) qualifying experience at the GS-5 level or equivalent , fully meeting all performance requirements for the GS-5 LPN/LVN.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

The full performance level of this vacancy is GS-06. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-03 to GS-06.
Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, pushing, and reaching over head. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements; such as annual TB screening or testing if required for the specific area they are assigned, as a condition of employment. Incumbent may be exposed, on a short-term basis to fumes, dust, and other debris. The employee must be able to occasionally lift and/or move 45 pounds or more. This employee must be able to carry 45 pounds' short distances. This position potentially requires flexibility in schedule and assignments, as this is a 24/7 hospital and is an essential function of the position. See VA Directive and Handbook 5019.
