The clinical pharmacy specialist (CPS) (Acute Care) works under their scope at the highest level of clinical practice to directly care for hospitalized patients. Acute Care CPSs round with the medical team, make recommendations on appropriate drug therapy based on patient specific factors; provide disease state management, and provide drug information for healthcare staff. Analyzes medication regimens to ensure the most-effective, least-toxic, and most-economical treatment plan. Designs, recommends, and implements pharmacotherapeutic regimens and corresponding monitoring plans. On a regular basis, meets with other medical staff to discuss patient medication therapy. Identifies those patients who are ready to be discharged and counsels patients regarding medications. Interviews patients to ascertain significant past medical history and medication history. As necessary, evaluates medication regimens for appropriateness, drug interactions, therapeutic duplications, non-compliance, and adverse drug events. Participates and contributes to educational programs (i.e. providing in-services/ staff education, serves as a preceptor for pharmacy residents and students). Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
