The Veteran-Directed Home and Community Based Services (VD-HCBS) Social Work Program Coordinator (SWPC) provides direct Veteran care services and is responsible for the implementation and support of the program. The SWPC will be responsible for day-to-day operations of the program and will oversee the program's effectiveness in meeting the objectives for VD-HCBS. The Social Work position is located onsite at the VA Long Beach Healthcare Facility.

Conduct psychosocial assessments, develops treatment plans in collaboration with the Veteran, relevant support systems and with the VA interdisciplinary treatment team. Offers psychosocial interventions and measures the outcomes interventions provided to Veterans. Identifies high-risk Veterans, providing crisis interventions and organizing case management / care coordination services. Assess and address presenting concerns and issues, coordinate Veteran / relevant support system conferences, and serve as liaison to all individuals & parties involved in care. Has extensive knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures; this includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. Responsible for establishing and maintaining ongoing effective therapeutic relationships with Veterans and their support systems. Coordinates community-based services, including information and referral for additional services from other VHA's, government, and community agency programs. Act as a liaison primarily to for Los Angeles and Orange Counties community organizations/agencies, the VA Long Beach HCS staff/treatment team and others as needed. Provides oversight to the program administered by the county community agencies, has regular oversight meetings, maintains an electric referral list, and maintains ongoing reports and data in support of the program. Manages the budget, oversees compliance with the VD-HCBS program Directive and provides financial information to leadership as requested. Other duties as assigned. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. [Transcripts must be submitted at time of application. Conferral of degree(s) must be stated on transcript(s). Unofficial transcripts(s) are acceptable] Licensure. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education: Applicants must have at least one (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. References: VA Handbook 5005/120, Part II Appendix G39 Social Worker Qualification Standard GS-0185 Veterans Health Administration. Preferred Experience: Experience working with Veterans, caregivers, all cohorts. Ability to do field work/work with community agencies. Ability to work across disciplines hospital-wide. Ability to oversee service related budgets/benefits. Physical Requirements: The SWPC must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Applicants must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation, if necessary, without endangering the health and safety of the applicant or others. The SWPC requires use of federal vehicles for the delivery of community-based services. The SWPC must be medically cleared and trained before operating a federal vehicle, pursuant to VHA Directive 2008- 020. All Social Workers who are required to use federal vehicles for their duties are REQUIRED TO BE CERTIFIED as an Incidental Driver. The SWPC may require a history review and if pertinent issues, may need to submit to a physical to be certified as an Incidental Driver. VA's are required to have current employee's and new hires who operate government vehicles and/or transport Veterans participate in periodic examinations every three - four years. As part of the certification, Incidental Drivers are required to be evaluated or examined as determined by the Occupational Health Examiner. There are no specific required lab tests however, during the medical history taking exam, the Examiner is required to evaluate any condition reasonably and adequately before providing the medical clearance. The position requires walking throughout the Medical Center and clinics, community agencies/court rooms, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams. This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.
