About the Position: Incumbent serves as a member of a multidisciplinary team, focusing on assessing, planning, implementing, coordinating, monitoring, and evaluating discharge and health care options and services to meet individual or family health needs.
This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DoD Workforce.
Duties
Coordinate and integrate the work of a variety of personnel to support safe and appropriate healthcare for all beneficiaries.
Provide professional and technical expertise to integrate facets of care for eligible beneficiaries to provide evidence based, patient centered care to provide the best clinical and business outcomes.
Order any or all durable medical equipment and supplies, as needed.
Provide education and/or clinical assistance to all medical staff.
Provide on-going education of the discharge planning process to patient and family members to maintain or improve their health status, with emphasis on a family-oriented prevention approach.
Identify issues with health care access and utilization in both government and private sector. Identify strategies to improve access to care.
Requirements
- This position requires the incumbent be able to obtain and maintain a determination of eligibility for a Tier 3 security investigation or access for the duration of employment. A background investigation and credit check are required.
- A Personnel Security Investigation is required.
- Selectees will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption.
- Immunization screening is required. Applicants may be required to show proof of other immunizations depending on the type of position.
- Hepatitis B immunization is required for all positions with direct patient contact.
- Applicants must be able to fulfill non-privileged professional credential requirements.

In order to qualify, you must meet the education and/or experience requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience. Basic Requirement for Supervisory Nurse (Consultant):
Degree: A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.).
Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
Foreign Degree: A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Applicants claiming this exemption must meet the requirement below. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status.
Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.
Active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Professional Nursing Experience: One or more full years of professional nursing experience which includes overseeing of one or more of the following: referral management, utilization management, case management, and disease management; providing technical expertise on evidence based, patient centered care to achieve optimal allocation of scarce medical resources for eligible beneficiaries; developing policy programs and/or guidelines for Medical Management using basic nursing and business protocols. This definition of professional nursing experience is typical of work performed at the next lower grade/level position in the federal service (GS-11).
Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted.
Education
FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
