This position is in the Patient Safety Office, which is under the Executive of High Reliability (EHR) Office at the Harry S. Truman Memorial Veteran's Hospital, Columbia, Missouri. The incumbent is responsible for assisting with the organization, coordination, implementation, and ongoing management of the Patient Safety Program as directed by the EHR and the Patient Safety Manager (PSM) and other patient safety duties as assigned.

Duties

Duties include, but are not limited to: Receives, categorizes, and applies severity assessment codes (SAC) to incident reports in the Joint Patient Safety Event Reporting (JPSR) reporting database, commensurate with potential and actual levels of patient injury Reviews appropriate CPRS patient information to clarify reports, level of injury, and/or interventions, to assure standardization of SAC coding application; enters de-identified incident reports into the national database; monitors and tracks incident reports entered into the JPSR system and ensures that investigators are in compliance with guidelines for completion of investigation into the incident report. Participates in Root Cause Analysis (RCA) cases as an advisor or member, using appropriate tools such as flow charting, planning matrices, brainstorming, and cause and effect analysis. Serves as a contact and/or advisor for RCA Teams. Assists with communication and monitoring of teams ensuring timely conclusion and submission of reports, and monitoring completion of action items and implementation plans from the RCA teams and relaying that to Medical Center Leadership, VISN leadership and the National Center for Patient Safety (NCPS) Monitors inpatient fall trends within the facility and reports to the appropriate committees and boards and reports this information to the Inpatient Evaluation Center (IPEC) monthly Develops reports including graphs and charts to display trending and tracking patterns pertaining to the Patient Safety Program and posts to the hospital Mission Control Board. Tracks resolution of problems identified in the Patient Safety Program and determines with the PSM staff education activities and remedial opportunities for improvement based on trends identified from analysis of data and recommendations made by RCA Teams. Conducts follow-up monitoring evaluation activities or special studies involving patient safety processes including data collection, analysis and validation as directed by the PSM to determine if remedial actions have been effective. Assists in the running of the Patient Safety Champion Program and facilitation of the facility Good Catch Awards program. Incumbent is the main point of contact for the Patient Safety Program including, but not limited to, day-to-day management of Patient Safety, including meetings, inspections, briefings, and presentations. Works with Services to schedule roundtables, RCA debriefs, forums and investigations and any follow-up post meetings. Provides administrative support to implement and maintain Medical Center policy and standard operating procedures (SOPs) for the Patient Safety Program to comply with VA and NCPS standards by ensuring local policies are up to date and compliant with national directives. Submits, formatted appropriately, through established procedures for the Director's signature when needed. Develops educational articles, storyboards and/or staff notices as guided by the PSM to impart program information to target audiences. Prepares and presents focused educational programs to employees at all levels in relationship to Patient Safety. Translates flowcharts into Healthcare Failure Mode Effect Analysis (HFMEA) worksheets and applies hazard score matrices to reflect priority focus areas. Work Schedule: Full time, Monday - Friday 8:00am - 4:30pm Telework: VA supports the use of telework as a way to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/18/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: providing technical or administrative support in clinical health professions student and resident programs, acting as a liaison, managing databases, onboarding processes, coordinating meetings, etc. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spend in such employment. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have a masters or equivalent degree or 2 full years of progressively higher level graduate education leading to such a degree or LL.B. or J.D., if related that demonstrates the knowledge, skills and abilities necessary to do the work of this position. (Transcripts Required). OR, Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond the first year of graduate level related education. (Transcripts Required). You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Physical Requirements: The majority of work is performed in an office setting; however, the incumbent will frequently use meeting room space and may visit all areas of the Medical Center as part of routine work.
