Employee will serve as a Social Work Associate aligned within Social Work Service at the Veterans Illiana Healthcare System in Danville, IL.

Duties

***THIS IS NOT A VIRTUAL POSITION, YOU MUST LIVE WITHIN OR BE WILLING TO RELOCATE WITHIN A COMMUTABLE DISTANCE OF THE DUTY LOCATION*** Duties will include, but may not be limited to the following: Develops psychosocial assessments of patients to identify social, emotional, physical, familial, economic, and vocational/avocational strengths, needs, and resources. Identifies community and other resources and facilitate linkage with and utilization of these resources by patients/families and caregivers. Follows-up to ensure resources/services were received. Makes referrals related to issues that are intangible in nature, including but not limited to: relationship conflicts, behavioral health, mental illness, substance abuse, aging (adjustment to changes in physical and cognitive functioning). Provides education related to benefits and services and referrals for other services/resources, including those beyond the scope of social work services, such as inability to meet basic financial needs, housing insecurity, transportation insecurity, questions related to charges for services and community provided medial services, and completing applications for various services/community resources. Participates on committees/work groups to represent social work services addressing program development and maintenance, direct service, education, or matters of an administrative nature. Qualifications

To qualify for this position, applicant(s) must possess all of the following qualifications: BASIC EDUCATION REQUIREMENT (TRANSCRIPT REQUIRED): Bachelor's degree (or higher) from an accredited college or university in major fields of study such as: social welfare, sociology, psychology, public administration, philosophy, education, anthropology, history, economics and/or English. AND SPECIALIZED EXPERIENCE: Two years of specialized experience as a social welfare worker or social work assistant in a voluntary, public, social, or health agency. One year of this experience must have been in a medical setting and equivalent to the next lower grade in Federal service. Specialized experience includes: developing psychosocial assessments of patients to identify social, emotional, physical, familial, economic, and vocational/avocational strengths, needs, and resources; identifying/providing community and other resources; provides education related to benefits/services and referrals; participating on committees/work groups to represent social work services addressing program development, education, matters of an administrative nature. AND ENGLISH LANGUAGE PROFICIENCY: In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position. You will be rated on the following Competencies as part of the assessment questionnaire for this position: CommunicationCustomer Service (Clerical/Technical)Technical Documentation IMPORTANT: A full year of work is considered to be 35-40 hours of work per week. All experience listed on your resume must include the month and year start/end dates. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. PHYSICAL REQUIREMENTS: The employee sits comfortably when completing clinical documentations. Other activities include standing and minimal lifting of less than 20 pounds. Requires a significant amount of walking throughout the medical center for face-to-face contact with patients, family members, community agencies, and medical center staff. May be required to push wheelchairs of patients with limited mobility when providing escort services, and the work may require special physical demands during therapeutic hold or therapeutic containment when responding to disruptive behaviors.
