The Oklahoma City VA Medical Center, Pharmacy Service, is currently recruiting for one (1) Clinical Pharmacy Specialist (Critical Care) in Oklahoma City, OK.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The Clinical Pharmacy Specialist, Critical Care is a representative of pharmacy service who will work closely with the medicine and/or surgery services in the medical ICU or the surgical ICU. The incumbent functions to provide for the pharmacotherapeutic needs of the patients. To fulfill these obligations, the incumbent has a comprehensive scope of practice, which includes prescriptive authority, monitoring drug therapy, ordering laboratory tests, and other specified clinical functions. The incumbent will also be responsible for developing, providing and supporting the educational programs of the clinical pharmacy section of the service, working with both the medical and pharmacy educational components of the medical center. The incumbent will assure compliance with all VA and Joint Commission requirements for medication management and appropriate monitoring, dispensing, and distribution of medications. Clinical pharmacist duties may be part of the position. Major Duties and Responsibilities: Clinical: Serves as an authoritative information source on drugs and their appropriate uses. Interfaces with physicians and other health care professionals in the care of patients to achieve rational, cost-effective drug therapy. Designs therapeutic plans, including the choice of medication, dosage, and therapeutic endpoints, based on patient specific information available from the medical record, patient or his representative, and physician using accepted pharmacokinetic concepts and pharmacodynamic principles. Education and Training Functions: Assesses and provides for the educational needs of the patient and family to help ensure that pharmaceutical outcome goals are met. Provides specialized education and training support to other professional services in the broad area of advanced pharmacotherapeutics. This may include medical students or medical residents. Serves as a clinical resource in the broad area of medication use in patients. Supervises undergraduate and graduate pharmacy students on assigned rotations if/when the facility has a relationship with a College/School of Pharmacy. Dispensing: Assures the accuracy and appropriateness of drug dispensing and compounding of medications based on patient history and medication profile review. This includes data entry into the computer, screening orders for drug-drug and drug-food interactions, dosing errors, allergies, incompatibilities, contraindications, formulary availability and final verification of the order. Consults with other health care professionals (i.e., physicians, nurses, dietitians, etc.) when necessary in all areas (i.e., Outpatient, Unit Dose, IV Admixtures, Ward Stock Pharmacy Services, CBOCs, State Veterans Centers, etc.) Consults with patients and medical center staff whenever there are questions concerning therapeutic or policy matters regarding outpatient prescriptions and inpatient physician orders. Provides quality control analysis of dispensing activities of automated dispensing equipment and pharmacy technicians. (4 Utilizes innovative and creative procedures to implement cost containment. Administrative Functions: Plans work and helps set priorities for the pharmaceutical care services and works with the Associate Chief, Clinical Pharmacy/Education and Program Managers of Primary Care and Inpatient Care to determine associate's clinical activities. Maintains an effective liaison with staff members of all professional disciplines and services for consultation regarding drugs, pharmaceutical problems, and pharmacy operating policies and procedures and works with the clinical coordinator and pharmacy supervisors to change procedures when necessary. Routinely informs the Associate Chief, Clinical Pharmacy/Education and Program Managers of Primary Care and Inpatient Care of any problems concerning the cost impact of changing prescribing patterns and offers recommendations to correct these problems to pharmacy and medical center administration. Coordinates and performs selected drug utilization evaluations at the request of the Pharmacy and Therapeutics (P&T) Committee. Serves as an active member of the medical center's committees as requested. Advancement of Profession: With approval of the Associate Chief, Clinical Pharmacy/Education and Chief, Pharmacy, may collaborate with members of the medical center staff on research projects as appropriate to the scope of this position with special attention to projects evaluating the clinical and economic impact of various therapeutic interventions. Basic Requirements: Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with 38 U.S.C. § 7407(a)). English Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f) Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) AND/OR, Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: ACPE NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. Grandfathering Provision: May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. GS-13 Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Clinical Pharmacy Specialist: The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following Knowledge Skills and Abilities (KSAs): Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: PGY1 Pharmacy residency; PGY2 residency in critical care Pharmacy or comparable. Physical Requirements: The position requires walking, standing, and sitting for long periods of time. The position also requires climbing stairs, lifting up to 50 pounds, dexterity of the hand and wrist, and arm strength to perform dispensing of medication orders.
