This position serves as a Nurse (Care Manager) in the Pawnee Indian Health Center ambulatory care clinic. The incumbent assists in the development of plan of care, ensuring that the plan is implemented on a timely basis and identifying patient needs. This position reports to the Supervisory Nurse of the ambulatory care clinic.

Duties

Promotes an interdependent collaboration with the patient, primary care provider, family, and other members of the health care team. Coordinates clinic visits for assigned patients in order to optimize care. Conducts pre-visit planning and communicates patient needs and recommends treatment of care team members. Serves as the primary contact/clinical liasion between referral hospitals, treatment facilities, and patients. Implements and coordinates specific caseload strategic interventions to move patients through the continuum of care. Provides direct nursing care and documents care in electronic health record.

You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S): Education:A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. AND Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: GS-10: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the GS-09 grade level in the Federal service obtained in either the private or public sector. Examples include promoting self-management of chronic disease through helping patients to identify barriers to making healthful choices; helping patient identify barriers to adhere to the prescribed regimen; providing coaching and guidance on positive health behaviors and providing chronic disease management; assessing, planning, implementing and evaluating the care of patients with medical, surgical, pediatric, newborn, obstetric, ambulatory and emergency conditions in the primary care clinic; providing chart reviews to identify patients at risk for increased morbidity and mortality and recommended interventions; providing patient and family education to assist in understanding and accept implications of care, diagnosis and treatment. OR Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
