This Job Opportunity Announcement will be used to fill a Social Worker - Suicide Prevention Coordinator (SPC) GS-0185-12 position in Mental Health Service at the Cheyenne VA Health Care System, Loveland, CO.

Duties

The Suicide Prevention Coordinator at the Cheyenne VA Health Care System (CVAHCS) is assigned to the Mental Health Service Line (MHSL). The incumbent is responsible for serving as Program Coordinator for the Suicide Prevention Program. The incumbent is administratively responsible for the Suicide Prevention Program in the Northern Colorado area. The incumbent will provide direct service in this major specialty area to Veterans, families, and caregivers, as well as providing consultation to interdisciplinary staff and facility leaders. The program coordinator oversees the daily operation of the Suicide Prevention program, develops policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use. The Suicide Prevention Coordinator functions include but are not limited to: Provides consultative services to other clinical and administrative staff, and to VA programs, focused on any aspect of suicide prevention. Provides supportive consultation in the development and utilization of Medical Center and community resources. Maintains lists of patients determined to be currently high-risk for suicide and monitor their care to assure that needed services are being received as well as maintaining a list of patients who are flagged for continual monitoring. Consults/collaborates with providers of high-risk patients for purposes of education and facilitating best practice and may optionally provide direct patient care for same purpose. Provides training to clinical and non-clinical staff in the Medical Center, Multi-Specialty Outpatient Clinics (MSOC's) and community about risk factors, warning signs, treatment options and resources. Promotes the use of evidence-based interventions and strategies for suicide prevention in the Medical Center to include the provision of Veterans Crisis Line (VCL). Serves on the hospital Suicide Prevention Committee, as well as other hospital committees and work groups related to patient safety and/or RCA (Root Cause Analysis) performing individual case reviews, assisting with preparation of quarterly aggregate summaries and facilitating recommended system improvements. Responds to consults from the VCL within 24 hours business days Suicide Prevention Hotline as soon as possible and contacts Veterans to engage them in treatment and reports back to the Hotline Staff about the results of interventions. Facilitates assessment of Suicide in the facility both by training staff, updating policies, and direct provision of assessment, according the VACO assessment guidelines. Facilitates implementation of reporting high risk patients through use of the Suicide Behavior Report. Facilitates the implementation of Level 2 Flags per Central Office Directives. Implements SAVE Training throughout Cheyenne Veterans Affairs Medical Center (VAMC), MSOC's and in the community. Acts as a liaison to community organizations and Veterans organizations. Provides Tracking Reports to VACO as required by the reporting guidelines. Consulting with providers on any no-show appointments by patients and following up with the patient as soon as possible to reschedule the appointment. Provides crisis intervention when indicated including evaluation, assessment, triage, initiation of support in Emergency Detention procedures, and disposition of patients experiencing behavioral health emergencies, Provides supportive consultation regarding transfer of psychiatric patients to other facilities with Psychiatric or Substance Abuse Inpatient Programs. May provide services through the use of Tele-Mental Health/Tele-Home technology. Qualifications

Applicants pending the completion of educational, or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: Citizenship: Be a citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. The following is the standard grandfathering policy for all title 38 hybrid qualification standards. Please carefully review the qualification standard to determine the specific education and/or licensure/certification/registration requirements that apply to this occupation. English Language Proficiency. Candidates must be proficient in spoken and written English. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level which includes: Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. The actual grade at which an applicant may be selected for this vacancy is GS-12. Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies such as papers, books, or small parts. The incumbent must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the applicant or others. The incumbent must be able to drive an automobile. This position also requires close visual acuity (near vision correctable at 13" to 16") to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers. Use of fingers, both hands required, ability for rapid mental and muscular coordination simultaneously, hearing (aid permitted), and clear distinctive speech are all required.
