This position is located at the White Earth Health Center in Ogema, MN. The position is located in the Purchased/Referred Care (PRC) Department. The primary responsibilities include clinical case management support to the Purchased/Referred Care Program. The incumbent of this position will report to the Administrative Officer or designee.

Duties

Serves as a clinical liaison and resource for the PRC staff for clarifying provider referrals/orders; assisting with the care coordination of ordering complex durable medical equipment (oxygen, power wheelchairs/scooters) and care managing mostly un-resourced patients/referrals but may include some resourced patients/referrals. Serves as the team lead for the outpatient Case Management and PRC referral database. Ensures the tracking processes of referrals are appropriate and meet patient centered medical home guidelines. Works with referral venders and other partners to ensure referral processes are appropriate and timely. Attends PRC Committee meetings as a resource for Medical Review members and as an advocate for the patient and their families. Other related duties as assigned.

This education must have been accredited by the Commission on Collegiate Nursing Education (external link), Council on Accreditation of Nurse Anesthesia Educational Programs (external link), Accreditation Commission for Midwifery Education (external link), or an accrediting body recognized by the U.S. Department of Education (external link) at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools. Areas of specialties for positions include, but are not limited to, Acute Care, Anesthesia, Community Health, Family, Geriatric, Home Health, Midwifery, Neonatal, Oncology, Pediatric, Psychiatric, Allergy and Immunology, Women's Health, Cardiovascular, Dermatology, Occupational Health, Orthopedic, Respiratory, or other specialties identified for the position. Licensure For all grade levels and positions, applicants must have passed the National Council Licensure Examination (external link). In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-09 and above grade level, many positions may require experience in a specialty area of nursing. Basic Requirements for the GS-11 (or equivalent) Grade Level: In addition to the above requirements, applicants must meet one of the following requirements: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Examples of Experience for the GS-11 Grade Level: Developing a tracking system for patient care coordination and care management across the continuum; independently plans and provides nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; providing case management for patients by coordinating services to meet patient's care goals; providing oversight to ensure compliance and appropriateness of care. Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
