This position is located at the Pine Ridge Service Unit in Pine Ridge South Dakota. The primary purpose of this position is to serve as a Utilization Review and Discharge Planning Nurse. Providing case management services needing coordination of care through the discharge planning services in the Acute Care Ward.

Duties

Applies case management essential activities assessment, planning, implementation, coordination, monitoring, evaluation, out comes across the continuum of care. Maintains current knowledge of resources available within the community, maintain supply of resource material to be distributed to patients when needed. Makes rounds of patient care areas to review charts for appropriateness, completeness, assess newly admitted patients and ensure timeliness of documentation by health care team. Compiles data and analyzes trends related to utilization of hospital health care resources. Communicates information related to trends and areas of improvement to stakeholders.

BASIC REQUIREMENT(S): Nurse Series 0610 (opm.gov) In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Developing a tracking system for patient care coordination and care management across the continuum; independently plans and provides nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; providing case management for patients by coordinating services to meet patient's care goals; providing oversight to ensure compliance and appropriateness of care. Time In Grade (Merit Promotion Candidates Only) Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
