This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

The Clinical Pharmacist Specialist in the Emergency Department has full time responsibility for providing pharmaceutical care for patients in the inpatient and outpatient areas to optimize patient response and resource utilization. The Clinical Pharmacist Specialist assumes the routine duties and responsibilities including but not limited to: Medication dispensing and distribution Inpatient/outpatient orders are checked to include patient information, correct medication dose, route, and frequency of administration. Orders are checked for patient allergies, drug interactions, incompatibilities, duplication, and priority. Identifies, interprets, and resolves prescribing and dispensing issues, including pharmacotherapeutic and the formulary. Documents interventions as necessary. Dispenses controlled substances with accuracy according to current policies and procedures for inpatients and outpatients. Clinical Serves to renew medications prescribed for chronic medical conditions if deemed appropriate after interviewing patient, interpreting laboratory values, patient response etc. Utilizes the highest level of clinical privileges for Clinical Pharmacy Specialists. Interfaces with providers, including physicians, PAs, ARNPs in the design of the veteran's treatment regimen with a focus on acute care management to achieve optimization of patient outcomes who present to the emergency department for care. Designs a therapeutic regimen or plan related to the patient's specific acute care issues, based on patient-specific information, which includes establishing a therapeutic endpoint, electing an appropriate drug and related therapy, designing an appropriate monitoring plan, and establishing an appropriate dosage regimen. Scope of Practice Works under individual Scope of Practice designated based upon the incumbents' education, training, and experience required to perform the functions of the position. Serves as an Advanced Practice Provider to initiate, modify or discontinue medication therapy. Will participate in a professional practice evaluation program. Education Participates in the training and orientation of pharmacy students, pharmacy staff, and allied healthcare professionals. Maintains and improves professional competency by keeping up with information on new medications, new medication therapies, and innovations in pharmacy practices. The incumbent continually applies current drug information and program techniques in their daily practices. Research Participates in research projects either by acting as a consultant or serving as a co-investigator and participates in the design, conduct and documentation of clinical drug investigations. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. NOTE: This applies to current/past/upcoming VHA HPTs only. An HPT is an individual appointed to a temporary position in one or more VHA medical facilities performing clinical or research training experiences to satisfy program or degree requirements or receiving VA Health Professional Scholarship Program. HPTs can be either VA-paid or Without Compensation (WOC). Basic Requirements: United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Pharmacists appointed to direct patient care positions must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part. Education. (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16 Grade Determinations: GS-13, Clinical Pharmacist Specialist Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Knowledge, Skills and Abilities (KSAs) - In addition to meeting the experience requirements for this grade level, the candidate must fully demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Completion of a PGY1 residency with learning experiences in emergency medicine and/or current direct patient care in emergency medicine. Physical Requirements: Typically, the employee can be expected to sit at a desk or table with walking and standing throughout the day. Items such as cases of drugs, nutritional supplements, prescription vials, totes, etc. are carried from delivery carts for placement on shelves. Lifting of moderately heavy objects approximately 40 lbs normally required.
