This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.


The Clinical Pharmacist Specialist - in the Patient Aligned Care Team has full time responsibility for providing pharmaceutical care for patients in primary care to optimize patient response and resource utilization. The Clinical Pharmacist Specialist assumes the routine duties and responsibilities below including but not limited to: Administrative Completes quality management data compilation for outpatient and inpatient clinical pharmacy activities as required. Serves as a consultant or as a member on various committees as needed and appropriate. Participates in treatment teams, committees and other groups needing input regarding the use of medication therapy or drug policy development as required. Assists in the formulation of policies and procedures within the Pharmacy and hospital policies that make reference to pharmacy programs as required. Clinical Direct Patient Care Activities Core functions include face-to-face and telephone encounters with referred patients, for comprehensive medication management in practice areas identified in their individual scope of practice. Functions as an Advanced Practice Provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Indirect Patient Care Activities Assists Pharmacy and Therapeutics committee with medication use evaluations and other safety initiatives. Promotes and monitors compliance for established drug therapy policies Scope of Practice Works under individual Scope of Practice designated based upon the incumbents' education, training, and experience required to perform the functions of the position. Serves as an Advanced Practice Provider to initiate, modify or discontinue medication therapy. Will participate in a professional practice evaluation program. Research Participates in research projects either by acting as a consultant or serving as a co-investigator and participates in the design, conduct and documentation of clinical drug investigations. Education Participates in the training and in-servicing of pharmacists, pharmacy technicians, pharmacy students, pharmacy residents, and other staff on assigned teams. Serves as preceptor for pharmacy students and pharmacy residents in accordance with established policies. Interfaces with other healthcare providers and administrators to ensure compliance with established pharmacy and medical center policies and procedures. Is skilled at using both written and verbal communication techniques in teaching others. Other duties as assigned. An HPT is an individual appointed to a temporary position in one or more VHA medical facilities performing clinical or research training experiences to satisfy program or degree requirements or receiving VA Health Professional Scholarship Program. Pharmacists appointed to direct patient care positions must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part. Education. (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16 Grade Determinations: GS-13, Clinical Pharmacist Specialist Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Knowledge, Skills and Abilities (KSAs) - In addition to meeting the experience requirements for this grade level, the candidate must fully demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Completion of a PGY1 residency with learning experiences in acute care, critical care, antimicrobial stewardship, infectious disease and/or current direct patient care in practice areas previously stated. Physical Requirements: Typically, the employee can be expected to sit at a desk or table with walking and standing throughout the day. Items such as cases of drugs, nutritional supplements, prescription vials, totes, etc. are carried from delivery carts for placement on shelves. Lifting of moderately heavy objects approximately 40 lbs normally required.
