This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
The Clinical Pharmacist Specialist - Acute Care has responsibility for providing pharmaceutical care for patients in the acute care setting to optimize patient response and resource utilization. The Clinical Pharmacy Specialist assumes the routine duties and responsibilities below including but not limited to:
Administrative
Responsible for the daily operation status of various aspects of the pharmacy programs. The incumbent prioritizes work and makes administrative decisions when supervisory pharmacists are not available, or in instances where immediate decisions are required. Pharmacy Service Policy and Procedures, VA Regulation and state and federal Pharmacy laws will serve as operational guidance.
Knows and follows administrative requirements of the hospital and those specific to the Pharmacy Formulary, investigational drugs, and general distributive functions.
Interfaces with other health care providers and administrators to ensure compliance with established pharmacy and medical center policies and procedures.
Serves as consultant and active participant in drug usage evaluation reviews. Assists in the development of proposals for improved and/or new clinical pharmacy services.
Is actively involved in facility n in areas of needs from a cost wide committees, workgroups, or other interdisciplinary groups of the like as required by the pharmacy department.
Participates in the review Program.
Clinical
Orders, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows-up with patients on lab or test results to monitor, evaluate, and adjust medication therapy.
Monitors serum levels for patients receiving drugs with narrow therapeutic indices and evaluates and adjusts medication therapy using pharmacokinetic and pharmacodynamic principles.
Ensures appropriate care coordination during transitions of care within the inpatient setting (changes in levels of care, shift changes, etc.) as well as throughout the discharge process, as indicated.
Scope of Practice
Works under individual Scope of Practice designated based upon the incumbents' education, training, and experience required to perform the functions of the position. Serves as an Advanced Practice Provider to initiate, modify or discontinue medication therapy.
Will participate in a professional practice evaluation program.
Education
Participates in the training and orientation of pharmacy students, pharmacy staff, and allied healthcare professionals.
Maintains and improves professional competency by keeping up with information on new medications, new medication therapies, and innovations in pharmacy practices. The incumbent continually applies current drug information and program techniques in their daily practices.
Obtains and maintains Basic Cardiac Life Support Certification (BLS).
Remains up to date on all Talent Management System (TMS) training.
Other duties as assigned.
Work Schedule: Monday-Friday, 7:00 am - 3:30 pm, rotating shifts to include weekdays, weekends, holidays and hours outside of designated tour of duty

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
NOTE: This applies to current/past/upcoming VHA HPTs only. An HPT is an individual appointed to a temporary position in one or more VHA medical facilities performing clinical or research training experiences to satisfy program or degree requirements or receiving VA Health Professional Scholarship Program. HPTs can be either VA-paid or Without Compensation (WOC).
Basic Requirements:
United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Pharmacists appointed to direct patient care positions must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part.
Education.
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16

GS-13, Clinical Pharmacist Specialist
Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
Knowledge, Skills and Abilities (KSAs) - In addition to meeting the experience requirements for this grade level, the candidate must fully demonstrate the following KSAs:
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards for their program area.
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Preferred Experience. Completion of a PGY1 residency with learning experiences in acute care, critical care, antimicrobial stewardship, infectious disease and/or current direct patient care in practice areas previously stated.
Physical Requirements: Typically, the employee can be expected to sit at a desk or table with walking and standing throughout the day. Items such as cases of drugs, nutritional supplements, prescription vials, totes, etc. are carried from delivery carts for placement on shelves. Lifting of moderately heavy objects approximately 40 lbs normally required.
