This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. The Michael E DeBakey VA Medical Center (MEDVAMC) is seeking a Social Worker - Prime Care-Mental Health Integration (PCMHI). This position will be located in Houston, TX.
Current permanent VA employee or Federal employee from another agency, and current/past/upcoming VHA Health Professions Trainee (HPT) Graduate, you should apply CBST-12129101-23-LG.
The social worker in this position it is expected to provide a full range of psychodiagnostic services including assessment and using the most appropriate psychotherapeutic techniques in providing quality care. The social worker applies principals of population based care (defines role and identifies problems rapidly). Initial appointments are based on functional assessments and treatment focuses on functional outcomes. The social worker is expected to provide brief mental health and behavioral interventions (e.g., 1 - 6 sessions for broad range of concerns) and use time-limited, evidence-based, interventions that are simple and concrete. The social worker provides a full range of therapeutic interventions including: individual and group therapies. The social worker is expected to use self-management/home-based practice interventions while also using brief measurement-based care routinely (e.g., PHQ-9, PCL, AUDIT C, etc). In addition, the social worker triages appropriately and efficiently to specialty mental health services.
The social worker will routinely consult with PCPs and other PACT team members about plan of care for patients, conduct effective curbside consultations and uses warm hand off strategy, and integrates within primary care team and work closely in collaboration with PACT teams. The social worker is responsible for attending PACT meetings and daily huddles. The social worker routinely provides feedback (verbal and/or written) to PCP and other PACT members same day as referrals are received. The social worker provides education to PACT on mental health and behavioral health issues as well as consultation on tips for successful care management and patient facilitation, even when there is no direct patient contact.
Social worker is able to coordinate community-based services, including information and referral for additional services from other VA programs, other government programs, and community agency programs.
Social worker possesses general knowledge of Veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension and facilitates referrals based upon Veterans' needs and eligibility.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure-Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
GS 11 Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates
Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. NOTE: For appointment licensure or certification at this level please refer to paragraph 3c.
OR
Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
(b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
(c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
(d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
(e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.

Preferred Experience:
Experience providing evidence-based psychotherapies including but not limited to: Ml/MET, CBT, ACT and experience providing measurement based care (PHQ9, GAD7, PCL, AUDIT C, etc.)
Working on an interdisciplinary mental health team
Experience providing individual and group psychotherapy
Adapting technology and computer assisted programs to deliver treatments and improve treatment outcomes.
Experience completing diagnostic mental health intakes.
The full performance level of this vacancy is 11.
