The incumbent serves as program coordinator at the GS-12 grade and is administratively responsible for the Contract Nursing Home (CNH) Program, which provides treatment to patients in a major specialty area. The incumbent provides direct patient care services and manages the daily operation of the program.The incumbent develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use.

The incumbent develops and implements information systems to track program activities including visits, referrals, admissions/discharges, inpatient days, patient demographics, and patient satisfaction. The incumbent collects, analyzes and reports program data to local management, and to the VISN and Central Office as requested. Evaluates program progress and outcomes and utilizes data to continuously modify and improve the program and processes. In addition, the incumbent uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. The incumbent performs clinical duties that involve specialized treatment of complex physical or mental illness. The incumbent incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. The incumbent is able to provide individual, group, and family psychotherapy and advanced level case management interventions used in the treatment of Veterans with polytrauma injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, addictions, and other serious disorders. The incumbent develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improved treatment services. The incumbent is able to demonstrate the ability to design changes based on empirical findings. The incumbent offers consultation to colleagues and students on the psychosocial treatment of patients treated in specialty areas, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills. The incumbent demonstrates the ability to teach and mentor staff and students in the special area of practice and may provide supervision for licensure or specialty certifications. The CNH Coordinator will be the primary Contracting Officer's Representative (COR) assigned to oversee existing nursing home contracts, and the acquisition of new contracts. COR responsibilities include oversight of contractor's deliverables, monitoring of contractor's performance, review and approve invoices, and provide technical direction to the Contracting Officer. The incumbent is responsible for scheduling and assuring that the yearly VA CNH program reviews are completed in the required time frames as set forth in VA policy and must have a good knowledge of Centers for Medicare/Medicaid standards concerning CNH programs. He/She is also responsible for assuring that monthly follow-up to Veterans in the CNH's are completed as required by the program outreach Social Worker and the RN assigned by nursing service to provide CNH follow-up. The incumbent is responsible for overseeing the placement of Veterans into the CNH program and assuring that the placement meets VA eligibility requirements. The incumbent is responsible for assuring that Quality Assurance monitors related to these programs are completed and reported to the CNH Oversight committee. The incumbent is responsible for reporting and investigating Sentinel/Adverse events to the local VA, VISN, and Central Office. The incumbent is the lead inspector for the annual inspection of the nursing homes. This includes coordination of the inspection dates, assembling of the team and education of the team members on the inspection process. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Senior Social Worker 0185, GS-12 Experience/Education: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. The actual grade at which an applicant may be selected for this vacancy is GS-12. Physical Requirements: This work is primarily sedentary but also requires some standing, walking, bending, lifting and carrying of items.
