The Chief Quality and Patient Safety Officer (CQPSO) Registered Nurse (RN) position is located in Network Directors Office within Veterans Integrated Service Network (VISN) 1 at one of the following campus locations: Bedford, MA, Boston, MA, Leeds, MA, Manchester, NH, Providence, RI, Togus, ME, West Haven, CT, or White River Junction, VT. Incumbent will be required to reside within VISN 1. This position is a full-time at 40 per week.
Duties
The CQPSO has the authority and responsibility for establishing quality and patient safety standards and ensuring these standards are met or exceeded and addressing expeditiously when they are not. The
CQPSO is the executive leader for regional healthcare markets in the continuous and timely availability of
safe, quality care Veteran services throughout the network. He/she is the principal executive for oversight
of Quality and Patient Safety throughout the Network and is the catalyst for the integration and collaboration of market quality/safety with other professional disciplines and functional areas in the mutual achievement of patient-centered and enterprise goals. He/She is the executive leader responsible for ensuring a High Reliability Organization with a culture of safety founded on a proactive systems-oriented approach to developing health care solutions with collective mindfulness. Programmatic oversight for quality management and patient safety services and may include other programs based on the regional market organizational structure, such as: Performance Improvement, Performance Measurement, Patient Safety, Accreditation, Risk Management, Internal and External Reviews, Utilization Management, Informatics, High Reliability, and Data Management.
The CQPSO is an executive leader within the regional network in alignment with the Office of Quality and
Patient Safety with strategic direction for the full continuum of care and quality operations. The CQPSO
has the responsibility to develop the regional market workforce strategies to ensure a strong workforce
that is prepared to meet the eight domains of quality competencies as well as advance the science of
improvement and innovation, and increase safety and reliability, which elevates the quality role, reputation
and practice of quality and patient safety locally, regionally, and nationally by establishing relationships
with educational institutions, national quality leaders, institutions for the science of improvement, and
professional quality organizations to draw talent to the VA, evolve practice models for quality and safety
professionals, risk managers, and certified professionals of health care quality, certified professionals of
patient safety, and certified professional in healthcare risk management, to transform health care and clinical processes and, provide oversight for regulatory, licensure and accreditation requirements related to all aspects of healthcare including quality and patient safety trainees.
As the Clinical Triad for the VISN with the Chief Medical Officer (CMO) and the Chief Nursing Officer (CNO), the
CQPSO will:
1. Advance health by providing expanded access to care with a focus on quality, safety, and
exceptional service,
2. Commit to delivering safe, high-quality, Veteran-driven care, and offering clinical expertise,
innovation, and cutting-edge technology and research to Veterans,
3. Advance the standardization for the implementation of Veteran health care and well-being, reduce
unnecessary clinical variation, based on the utilization of best practices supported by research and
evidence-based care.
The CQPSO, in collaboration with other VISN leaders will:
1. Lead, oversee and direct the VISN quality, safety, and improvement programs to assure
coherency, and consistency with network and system-wide goals and strategic objectives,
including VHA' s strategic framework for quality.
2. Monitor and evaluate performance and quality and safety of care across the network.
3. Develop, facilitate, and implement the foundational strategy, change management, and innovation that is necessary to ensure growth and sustainability with the highest level of quality, safety, and
service, ultimately resulting in a network-wide reduction of variance in quality/safe patient care.
4. Lead organizational change required to achieve quality, safety, and improvement goals, including
identifying and facilitating resolution of obstacles to achieving these goals.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience:
Five (5) plus years of leadership experience in Quality Management
Certified Professional in Healthcare Quality (CPHQ) strongly preferred
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse V - Master's degree in nursing or related field with BSN and approximately 5-6 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and appropriate basic or advanced certification desired, and approximately 4-5 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: The work is mostly sedentary. Typically, the incumbent sits comfortably for the work. However, there may be some walking; standing; bending; carrying of light items, such as papers, books, or small parts; or driving an automobile. No special physical demands are required to perform the work. The incumbent will be required to travel occasionally.
