The Oklahoma City VA Medical Center is currently recruiting for Clinical Pharmacists to work with the Pharmacy Service. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations.

Duties

The duties of the Clinical Pharmacist include the following, but are not limited to: Dispensing: Assures the accuracy and appropriateness of drug dispensing and compounding of medications based on patient history and medication profile review. This includes data entry into the computer, screening orders for drug-drug and drug-food interactions, dosing errors, allergies, incompatibilities, contraindications, formulary availability and final verification of the order. Consults with other health care professionals (i.e., physicians, nurses, dietitians, etc.) when necessary in all areas (i.e., Outpatient, Unit Dose, IV Admixtures, Ward Stock Pharmacy Services, CBOCs, State Veterans Centers, etc.) Consults with patients and medical center staff whenever there are questions concerning therapeutic or policy matters regarding outpatient prescriptions and inpatient physician orders. Provides quality control analysis of dispensing activities of automated dispensing equipment and pharmacy technicians. Utilizes innovative and creative procedures to implement cost containment. Clinical: Provides drug information to physicians, nurses, and other associated health care professionals. Provides oral and written drug information to all patients. Participates in pharmacy-based drug information services. Participates in the pharmacy and medical center s quality assurance activities. Participates in the medical center's drug use evaluation program. Authorized to provide partial fill of prescriptions under written protocol. Education: Acts as clinical institutional instructor for professional or technical level pharmacy students working in the service as a hospital internship training site. Participates in educational programs for medical center staff and pharmacy drug education activities. Assists in the training of staff (i.e., new providers, technicians, pharmacists, etc.). Work Schedule: Full-Time. This position may be required to rotate through the following tours including, but not limited to 7:00am-3:30pm, 8:00am-4:30pm, 10:30am-7:00pm, 12:30pm-9:00pm, 3:30pm-12:00am (midnight) or any other tour deemed appropriate based on the needs of the service. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Experience or Education. In addition to the basic requirements, candidates must meet one of the following: 1 year of experience equivalent to the next lower grade level, or Completion of an ACPE-accredited Pharm.D. program. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): 1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
