The Social Worker (Program Coordinator) is assigned to VA Community Nursing Home (CNH) Program as a Supervisory Social Worker to manage the administrative and clinical aspects of the Community Nursing Home Program that is located across the VA CCHCS, Fresno, CA and catchment areas.

Duties

The Supervisory CNH Social Worker manages the daily operations of the program and direct patient care services in the program area. Incumbent develops policies and procedures for program operation and prepare reports and statistical data for the facility, VISN, and National use as required. Incumbent is responsible for the program's fiscal operations and various fund control points assigned to the program. The incumbent provides direct clinical services through visitation to Contract Nursing Home residents and coordinates conferences with facility staff to address and resolve patient concerns as they develop and ensures services are provided with attention to dignity of resident and age-related capacity to understand treatments per our CNH National Directive/Policy. Incumbent is assigned a caseload and is responsible to monitor and provide case management and coordination to Veterans enrolled in the CNH Program. Major duties include, but are not limited to: Assuring that the placement of Veterans into the CNH program meets VA mandatory eligibility for care in a nursing home as defined by the CNH Program National VHA Directive. Assuring the reporting and investigating of sentinel/adverse events by program staff as necessary. Performing work related to the review, monitoring, analysis, planning and assessment of various programs. The incumbent is responsible for key project activities related to the planning, development, solicitation, and implementation of the project component as a Contract Officer's Representative (COR). Providing leadership, guidance and direction for the clinical and administrative findings of the CNH Program. Develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Supervising a group of employees performing work at various GS levels and provides administrative, technical and clinical supervision necessary for accomplishing the work of the unit. Reviewing all referrals to the CNH program and determines appropriateness for services. Serves as the primary contracting officer representative for all CNH contracts. Budgeting and tracking of all expenditures for the Contract Community Nursing Home Program. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. Grade Determinations: Supervisory Social Worker, GS-12 Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities:, In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Ability to independently organize work, set priorities, and meet multiple deadlines. (b) Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. (c) Ability to ensure provision of clinical social work services. (d) Ability to provide staff training and development. (e) Skill in interpersonal relationships in dealing with employees, team leaders, and managers. References: VA handbook 5005 Part II Appendix G39The full performance level of this vacancy is GS-12. Physical Requirements: Physical demands of the work are generally minor and are generally sedentary in nature but may require occasional travel.
