The North Florida/South Georgia Veterans Health System is recruiting for a Registered Nurse Performance Improvement Home Based Primary Care (HBPC) to work at the Malcom Randall VA Medical Center within the Home Based Primary Care Service. Duties
The Home Based Primary Care (HBPC) Registered Nurse Performance Improvement is responsible for all elements of the nursing process when providing direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. The RN HBPC performs diverse nursing functions such as significant case management, coordination of patient care, physical assessment, creating care plans, completing primary care clinical reminders, teaching in-home patient care to Medical Foster Home (MFH), family caregivers.
This position consists of 0.50 as a Registered Nurse (RN), Home Care/Home-Based Primary Care (HBPC)and 0.50 as a RN, Home Care/Performance Improvement. In addition to being a core member of the HBPC interdisciplinary team, the HBPC Registered Nurse functions in the PACT team role as a care manager and is responsible for:
Functions as a registered nurse in the Veteran's home environment.
Develops and coordinates the patient's plan of care to include ongoing assessment and re-assessment, care planning, care coordination, monitoring, and evaluating clinical outcomes.
Evaluates Veteran's health, family, and psychosocial history to identify health care needs and initiates or revises plan of care based on polices of HBPC.
Performs physical examinations, using nursing techniques of inspection, palpitation, percussion, and auscultation.
Uses appropriate instruments, orders and performs relevant diagnostic tests.
Provides health teaching to Veterans and their families related to present health problems, prevention of illness, accidents, and health maintenance for them to assume more responsibility for their overall health care and to optimize clinical outcomes and support quality of life decisions.
Interacts, coordinates and advocates for the patient/caregiver with other relevant parties across a continuum of care, beyond a single episode of care, ensuring seamless transitions.
Provides intensive care management which may include educating veteran on medication and treatments; initiates referrals, consultations, community follow ups and equipment and supply orders as appropriate.
Links Veterans with community resources.
Coordinates resources for comprehensive care to avoid duplication of services and streamline staff interaction and services with the Veteran.
Participates in the development of the individual plan of care at interdisciplinary care meetings for assigned panel of patients.
Works along with the APRN and other disciplines as part of the HBPC PACT team to provide care to a panel of his/her patients.
This position requires a great deal of driving and very significant case management duties.
The role may support the provision of care by others to the HBPC population, Medical Foster Home and Veteran Directed Care populations.
This position may also help with patient care needs in other areas of Home Care/HBPC as needed, or other duties as assigned by the Associate Chief of Staff for Geriatrics and Extended Care &/or the Chief, Home & Community Based Care.
The RN, Home Care/Performance Improvement position is responsible for the Performance Improvement Program of all Home Care Services of Geriatrics & Extended Care. The RN is responsible for the performance improvement program and compliance with Joint Commission Accreditation Standards, Veterans Health Administration National Patient Safety Standards, and compliance monitoring of Home Based Primary Care, Medical Foster Home, and clinical review and oversight of need for Home Care programs.
The HBPC Registered Nurse is required to travel to other locations of care within North Florida/South Georgia Veterans Health System (NF/SG VHS) to carry out job duties/assignments in patient homes. The RN is required to drive and/or travel using personal vehicle or government vehicle to other sites of care within NF/SG VHS, as well as, to community partners, as necessary to accomplish work assigned. Travel will be for long periods of time, up to three hours each way; travel is expected up to 75% of work week.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires long periods of continued standing (1-3 hours), frequent moving and lifting of patients and equipment, stooping, sitting, bending; pulling and pushing. Transferring patients and objects may be required. This position is required to operate a Government-owned or leased vehicle to properly carry out assigned duties, as needed (motor vehicle incidental operator).
Pre-Placement Examination Requirement: All Motor vehicle operators and incidental drivers shall undergo a standard pre-placement examination and periodic medical evaluation at least once every four years to ensure that employees who operate government owned or leased vehicles are medically able to do so without undue risk to themselves or others. Note: To qualify as an incidental operator an employee must possess a valid state license, have a safe driving record, pass a road test, and demonstrate that the or she is medically qualified to operate the appropriate motor vehicle safely.
