The Kinesiotherapist is a clinical specialist under Physical Medicine & Rehabilitation/Transitional Care & Rehabilitation functions in the role of an independent clinician while serving as an integral member of the Home-Based Primary Care Program/Foster Care team. This health care professional is responsible for evaluating referred patients and independently establishing and executing treatment plans that include a full range of complex treatment procedures and modalities.

Duties

Duties included but not limited to : FUNCTIONS OF THE POSITION: HOME-BASED PRIMARY CARE/FOSTER CARE PROGRAM 1. Patients are evaluated in such a manner as to not negatively impact other clinical staff yet meet the patient's needs. Coordinates specialty skilled assessments in the patients' home setting with other Home-Based Primary Care team members. 2. Patient medical records are reviewed to assess appropriateness of Kinesiotherapy intervention; to identify potential risks or contraindications to treatment; and to provide assistance in guiding the evaluation process. 3. The patient needs are frequently complex and may need to be adapted because of physical disabilities or multiple medical conditions. Evaluations are conducted without consultation with other therapists. Evaluations include medical history, results and interpretation of appropriate functional tests and measurements such as: range of motion, muscle strength, sensory/perceptual deficits, coordination, functional mobility, pain scales, and social needs. 4.Evaluations address rehabilitation potential, behavior, motor skills, and psychosocial development at an age appropriate level. Barriers to learning are identified and strategies to overcome them developed and implemented to ensure accurate patient evaluation. 5. Required assessments for the Home-Based Primary Care Program including Safety and Functional Assessments are completed on a timely and accurate basis. Reassessments are completed annually on enrolled patients. Skilled care needs are independently assessed and recommended to the Primary Care Physician. Independently schedules assessment and treatment for identified patients. Evaluates and treats under Physician's orders. 6. Evaluation findings are used to develop and implement comprehensive patient treatment plans including objective, measurable goals and timelines for completion. These goals are developed in collaboration with the patient, family and/or stakeholder input. 7. Adapts and modifies treatment plans, activities, and procedures to meet the needs of patients with complex disabilities. Documents treatment progression and/or modifications according to PM&R Program, Medical Center and JCAHO standards. 8.Responsible for Hurricane evacuation plan of HBPC patients and attends all meetings and discussions with counties emergency operation office. 9. Responsible for home safety evaluations and setting up adaptive equipment in the home. This requires the therapist to train the caregivers. The therapist is also responsible to teach patients safety precautions when using oxygen in determine if needed by monitoring the patient's ox pulse. 10.Ensures appropriate patient referrals and discharge planning in concert with Primary Care, PM&R and other Medical Center staff. 11.Ensures adequate coding and documentation within timeframes to support Medical Care Cost Recovery billing in support of Medical Center reimbursement, revenue streams and Home-Based Primary Care program goals. 12.Provides patient care which is appropriate to the cognitive, emotional and chronological maturation needs of the adult or geriatric and/or geriatric patient. The incumbent must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients serviced in his or her assigned service area. 13.Develops all fire evacuation plans in the home and instructs patients and caregivers on use of and placement of smoke detectors and fire extinguishers. 14.Investigates current concepts and scientific principles in Kinesiotherapy practice through literature reviews, study of current research, clinical practice, assisting in research trials, and through attendance at continuing education courses and conferences. 15.Serves as Clinical Instructor for any assigned student, and/or Assistants. 16.Consults with staff and resident physicians and other Home-Based Primary Care staff through patient presentations, attendance at interdisciplinary meetings, patient home rounds, specialty clinics and/or conferences. 17.Maintains Kinesiotherapy, PM&R, and Medical Center competencies, participates in peer review activities and supports all Quality Improvement activities and programs for the Medical Center. 18.Serves as the subject matter expert in the specialty or program area and as a consultant to Kinesiotherapy and other medical center staff in the areas of patient evaluation and treatment.19.Demonstrates the ability to identify and resolve interpersonal or professional conflicts when dealing with patients, caregivers, members of the treatment team and all Medical Center programs or services. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Kinesiotherapist candidates must be proficient in spoken and written English to be appointed as authorized by [38 U.S.C. ? 7403 (f). Education: (1) A bachelor's degree [or higher from a program accredited by the Commission on Accreditation of Allied Health Education Program, with a concentration/emphasis in kinesiotherapy.VA bachelor's degree [or higher from a program accredited by the Commission on Accreditation of Allied Health Education Program, with a concentration/emphasis in kinesiotherapy. OR (2) A bachelor's degree or higher with a concentration/emphasis in kinesiotherapy or exercise science related field and registered as a kinesiotherapist in good standing with the COPSKT. OR (3) A master's degree or higher with a degree in exercise science, kinesiology, human performance or a related field, which has been approved by COPSKT and a certificate verifying completion of a Center of Excellence (COE) Kinesiotherapy clinical training program. The completion of the certificate ensures that COPSKT has approved the degree program as part of the application process for the COE internship. Foreign Graduates. Graduates of a foreign program must meet the requirements in subparagraph 3b if they have a full unrestricted and current registration to practice kinesiotherapy in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States or in the District of Columbia. They must have at a minimum, proof of a degree from an accredited college or university (or foreign equivalent, as verified through an independent credentialing evaluation company). Registration. All applicants must be registered in kinesiotherapy in good standing with the COPSKT at the GS-11. Loss of Credential. Registered kinesiotherapists who fail to continue their registration in good standing, and maintain the required registration, must be removed from the occupation, which may result in termination of employment. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-11:Specialized Experience/Education: One year of experience equivalent to the next lower grade level (GS-09) OR Ph.D. or equivalent doctoral degree or three academic years of progressively higher-level graduate education leading to a degree in kinesiotherapy or an exercise science related field. AND Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSA's: Skill in communicating and coordinating with patients, families, caregivers, and other health care professions to facilitate the multidisciplinary treatment process. Knowledge of evidence-based kinesiotherapy to provide skilled rehabilitation interventions to a wide variety of patients having diverse and multiple disabilities. Ability to conduct independent assessments using highly specialized evaluations and diagnostic tests. Knowledge of [learning principles, behavioral intervention, and innovational] techniques for patient and caregiver education. Ability to adapt assessment tools and treatment to the complexity of the diagnosis or disabilities and demonstrate the clinical reasoning necessary to identify the need for further - depth specific assessment of function and utilization of non-standard methods and techniques. Ability to use clinical reasoning necessary to identify the need for further in-depth specific assessment and/or continuation of treatment. Preferred Experience: Geriatrics or home care experience.
