The Clinical Pharmacy Practitioner (CPP) is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. The CPP functions at the highest level of clinical practice and works independently under their scope of practice as defined by the Veterans Health Care System of the Ozarks to directly care for patients.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Duties include. but are not limited to: Clinical Functions This CPP supervisor position will be 50% clinical providing appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate. The CPP will provide these services while functioning under a Pharmacy Ambulatory Care Scope of Practice. The Scope of Practice for a CPP must allow for initiation, modification, and discontinuation of medications based on the principles previously listed Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing. Monitoring will include prospective review and intervention in: a. Therapeutic appropriateness of a patient's regimen. b. Therapeutic duplication in the patient's drug regimen. c. Appropriateness of the route and method of administration. d. Degree of patient compliance with the prescribed drug regimen. e. Drug-drug, drug-food, drug-laboratory, or drug disease interactions. f. Clinical and pharmacokinetic laboratory data to evaluate the efficacy of drug therapy and to anticipate side effects, toxicity, or adverse effects g. Physical signs and clinical symptoms relevant to the patient's drug therapy Performs continuous evaluation of prescribed medications to assure optimal drug therapy. Provides medication counseling to patients. Completes medication histories including patient interviews. Supervisory Controls The Pharmacy Supervisor Home-Based Primary Care (HBPC) reports administratively and clinically to the Associate Chief of Clinical Pharmacy with the Veterans Health Care System of the Ozarks (VHSO). The Pharmacy Supervisor (HBPC) serves as the frontline manager for the Pharmacy HBPC CPPs and plans and carries out the duties of a supervisor independently. Including but not limited to approving/disapproving time and leave requests, completing e-Performance evaluations, and reviewing OPPE/FPPE. Responsible for developing and managing clinical pharmacy activities and functions with an emphasis on improving quality of medical care for patients; reduce the cost of treatment and hospitalization; and assure safe and rational use of medications. Participation in documentation of interventions, medication error and adverse drug event reporting. Assists with resolving JPSR complaints regarding Pharmacy. Works with other health care providers to develop programs appropriate to incorporate PACT CPPs into Primary Care for improvement of drug use through comprehensive medication management. Works in conjunction with the Associate Chief of Clinical Pharmacy to develop and conduct medication usage evaluation in areas of need from a cost and patient outcome perspective. Documents cost reduction/cost avoidance activities on a periodic basis. Reports data to the Associate Chief, Pharmacy Service quarterly. Educational Participates in self-development activities to maintain and increase professional competence. Identifies potential candidates for positions of higher responsibility and assists in their HPDM related developmental experiences. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Pharmacy Supervisor must be proficient in spoken and written English as required by 38 U.S.C. 7403(f). Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Pharmacy Supervisory, GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Demonstrated Knowledge, Skills and Abilities: Pharmacists assigned to this position must demonstrate the following KSAs: - Ability to effectively supervise subordinate staff. - Ability to communicate orally and in writing to persuade and influence clinical and management decisions. - Expert understanding of regulatory and quality standards pertaining to pharmacy. - Skill in managing people or programs. Preferred Experience: At least 2 years working as a HBPC Clinical Pharmacist Practitioner. References: VA Handbook 5005/55, Part II Appendix G15, Licensed Pharmacist Qualification Qualification Standard. This can be found in the local Human Resources Office. The actual grade at which an applicant may be selected for this vacancy is GS-13. Physical Requirements: Physical Demands: The Pharmacy can be an exceptionally stressful work environment. Good mental and emotional health is required in order to function at an optimum level. Work areas can be high stress environments, due to volume of workload, which must be accomplished. Patients may sometimes be difficult. The position requires a high level of conduct, appearance, and general courteous manner which facilitates dealing with patients, physicians, and nurses in an effective way. Work Environment: Work is performed in patient care, office, and storage areas of the facility. Work areas are adequately heated and lighted. Potential exists for: exposure to infectious patients or materials; exposure to hazardous and toxic substances including but not limited to cytotoxic chemotherapy, solvents, grease, oils, and cleaning solutions; sticks or cuts from hypodermic needles, lancets, or other sharp items; musculoskeletal injuries if proper lifting and carrying techniques are not used; unusual musculoskeletal fatigue due to prolonged standing, electronic data processing, etc.; eye strain.
