The Clinical Pharmacy Specialist (CPS) functions at the highest level of clinical practice and top of licensure and serves as an advanced practice provider working independently with prescribing privileges as outlined in the individual's practice area-based scope of practice. The CPS provider delivers evidence-based comprehensive medication management (CMM) through direct patient care and as part of the interdisciplinary team providing Pain Management.

Duties

Duties include, but are not limited to: Prescribe, monitor, and adjust medications related to pain management and obtain accurate pain medication related history from patient(s) and via chart review. Perform rating scales on pain control, symptoms, and functional status. Use available population health tools to identify and address risk mitigation needs focused on safe pain care. Provide input and recommendations as the pain medication expert on the interdisciplinary pain management team during routine patient care planning or targeted complex or high-risk reviews. Review relevant population management dashboards to identify patients who may benefit from engagement in care for pain management and/or risk mitigation. Perform limited external physical assessments and perform medication reconciliation in alignment with established policies or procedures. Provide CMM for complex patients with chronic diseases including, but not limited to, alcohol misuse/use disorder and tobacco misuse/use disorder. Order and interpret labs or other needed tests pertinent to CMM and make needed referrals. Review patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Provide education to patients, their families and caregivers about medications, patient responsibilities, and information on health promotion. Participate in the appropriate, effective, and safe use of pharmaceuticals by contributing to criteria development, data collection, quality assurance and drug use evaluation and other quality improvement activities for core practice area(s) and pain management. Work Schedule: Monday-Friday, 8:00am-4:30pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL IBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. GS-13 Grade Determinations: In addition to the GS-12 requirements, must have one year of experience equivalent to the next lower grade level. Demonstrated KSAs. The KSAs shown below apply to all assignments at this grade level. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. (KSAs must be clearly reflected in resume under description of duties in order to be found qualified for this position/grade level). Preferred Experience: Preferred at least one year of experience in pain management. References: VA Handbook 5005/55, Part II Appendix G15, Licensed Pharmacist Qualification Standard. This can be found in the local Human Resources Office. The full performance level of this vacancy is GS-13. The actual grade at which an applicant may be selected for this vacancy is GS-13.
