Duties
The Management and Program Analyst provides support to the Primary Care Service at a VA
Medical Center or Health Care System and related Community Based Outpatient Clinics (CBOCs)
within the Veterans Integrated Service Network (VISN) 15. The primary responsibility of the position
is serving as the principal facility coordinator (PFC) of the Patient Centered Management Module
(PCMM) and Newly Enrolled Appointment Requested (NEAR) Coordinator for all Patient Aligned
Care Teams (PACT) within Primary Care Service. The incumbent serves as the subject matter
expert providing high-level operational oversight and expertise of the PCMM web application and
NEAR database.
The PCMM is a complex, nationally implemented, stand-alone computer program that is designed
to monitor and manage patients and staff in primary care, mental health, and home-based primary
care services throughout the VA system. The PCMM Principal Facility Coordinator (PFC) is the
facility's designated staff member who manages and maintains the access, data entry, and
accuracy of the PCMM. The PFC provides management and oversight of the PCMM web
application, ensuring that the data entered into PCMM is reliable and consistent; adheres to policy
and business rules; and for data management, education, and training. The PFC works with clinic
support staff, clinic managers, clinical service chiefs, CBOC staff, Administrative Officers, MCAA
staff, VISN staff and national staff to maintain the integrity of the PCMM application, functionality,
utilization, and resulting data. These duties include monitoring panel sizes, oversight, and
correction of transmission errors, and review/rectification of incorrect patient assignments.
Provides input to VISN and national program managers on process challenges, program issues,
and other problems so these items can be addressed, corrected, and/or improved.
Establishes and manages the PCMM PACT position set up, staff and FTE assignment, as well as
patient assignments to clinics. Specifically, the PFC electronically creates or modifies team and
position settings, assigning provider user class based on verified credentials, links the associated
clinic(s), sets maximum panel sizes, programs clinical alert notification levels, and establishes
preceptors to associate provider links. Additionally, the incumbent uses multiple
assignment/reassignment tools to move large groups of PC and /or MH patients to different teams
and positions when changes become necessary. Meets regularly with facility Primary Care
leadership and group practice managers to review provider panel management data to ensure
patient assignments are made according to the current accessibility of the clinics and patient wait
times, including access for face-to-face and telephone care. Patient assignments are made to the
lower capacity clinics to ensure patient wait times are less and care is completed in a timely manner.
Manages the automatic unassigned patients by running VSSC reports and communicating with
the Primary Care clinics to ensure the patients are contacted. Assures that Change of Provider
forms are completed timely.
Serves as the facility Newly Enrolled Appointment Requested (NEAR) Coordinator managing the
NEAR List and process; provides training on NEAR process flows. Works closely with NEAR team
members to appoint new enrollee appointment requests in accordance with VHA Directive 1230.
Facilitates a warm transfer from enrollment to the NEAR schedulers for eligible Veteran walk-in
enrollments who desire an appointment before leaving the facility. Ensures scheduling staff have
updated lists of which providers schedule new Primary Care patients which prevents assignment
of too many pending patients to the panel. Assigns new, established, re-established patients before
provider appointment occurs or no later than two business days of provider appointment. Enters
chart notes and alerts the new PACT team of patient assignment to a new clinic and asks the team
to contact the patient for a new appointment.
Serves as the Primary Care Automated Data Processing Application Coordinator (ADPAC) and
is responsible for completing and assisting with system access requests through ePas. Trains
new users on their system access packages in VISTA and using the various function keys to
ensure reports are ran and access to appointment management is available.
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 09/15/2023.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05 and for a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. GS-07: SPECIALIZED EXPERIENCE: To be eligible for this position at the GS-07 level you must possess at least 1 year of specialized experience equivalent to the GS-05 level. Specialized experience is experience that equipped the applicant with the particular knowledge, skills, and abilities (KSA's) to perform successfully the duties of the position, and that is typically in or related to the position to be filled. In this particular position, examples of specialized experience include but are not limited to:
Experience monitoring provider panel sizes
Experience assisting manager with implementing organizational improvement
Review established local policy to ensure it is in accordance with VHA Directives
Performing audits
Experience with ViSTA and function keys

EDUCATION: 1 full year of graduate level education that demonstrate the knowledge, skills, and abilities necessary to do the work. One year of full-time graduate education is the number of credit hours the school attended has determined to represent 1 year of full-time study. If that information cannot be obtained from the school then 18 semester hours or 27 quarter hours should be considered as satisfying the 1 year of full-time study requirement.
