The Marion VA Health Care System is seeking a full-time Care Coordination and Integrated Case Management, Registered Nurse within Social Work Services located in Marion, IL or Evansville, IN. The CC&ICM will partner with the CC&ICM Social Work Coordinator to implement the national CC&ICM framework which serves a patient population with highly complex health and mental health problems to ensure they receive the right level of care coordination support based on their prominent need.

Duties

The incumbent has both clinical and administrative responsibility to ensure effectiveness of services, application of latest VHA guidelines and modification of service patterns. The incumbent will provide coordination and direct clinical evidence-based services including, assessments and interventions; program development; Veteran, family, caregiver and staff education on Care Coordination (CC) and Integrated Case Management (ICM); resource development; community outreach; continuous quality improvement activities, and evaluation/consultation. The incumbent is administratively responsible for the clinical programming and daily operations of the Marion VA Health Care System (HCS) CC&ICM program. Develops and implements policies and procedures specific to Marion VA HCS and ensures compliance with local facility, VISN, Office of Nursing Service (ONS) and Care Management and Social Work (CMSW), and VA Central Office directives and handbooks, as well as the Joint Commission, and other accrediting bodies and regulatory standards. The incumbent manages and controls the use of data related to these programs regarding day-to-day functioning and program evaluation. In conjunction with the Service Chief and/or supervisory designee and nursing counterparts, plans, organizes and directs administrative, operational, Veteran and personnel activities for the service. The responsibilities that fall under the RN CC&ICM Program Coordinator include but are not limited to the following tasks: - Identify cohort of Veterans across the Marion VA HCS with the highest opportunity and highest need. - Identify potential areas where care coordination is taking place to include but not limited to primary care, surgical care, mental health, community care, specialty clinics, geriatrics, and chronic disease. - Integrate inpatient and outpatient care coordinators, care managers, and case managers for seamless continuity of care. - Develop, implement, and monitor process for ongoing screening of identified cohort. - Identify care coordinators and support services across the specialized services of Marion VA HCS and ensure continued participation by setting expectations, clear roles and responsibilities. - Develop and implement facility wide CC&ICM education plan to key stakeholders to include Service Chiefs, supervisors/managers, frontline staff, and Veterans with plan for ongoing sustainment education plan. - Coordinate and collaborate with the facility level care coordination and case management teams to include but not limited to nurse and social worker navigators, care coordinators, care managers, and case managers. - Coordinate and co-facilitate a care coordination review team (CCRT) to integrate communication, collaboration, and coordination when assigning a lead Coordinator to Veterans identified as needing moderate or complex care coordination services. PLEASE NOTE: These positions require Basic Life Support (BLS) certification. If selected, Applicants will provide proof of current certifications prior to employment. Only BLS certification which included Automated External Defibrillator (AED) obtained from either the American Heart Association (AHA) or the Military Treatment Training Network (MTN) will be accepted. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: BSN preferred, two (2) years of Case Management preferred, and program coordination/development preferred Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Able to communicate effectively with Veterans and staff via phone and in person (with or without adaptive equipment), Unusual fatigue factors (emotional stress), Protracted or irregular hours of work, Work closely with others, Light lifting, under 15 pounds, Light carrying, under 15 pounds, Prolonged close computer work, Prolonged work in surgical mask, Reading and writing (prolonged periods with or without adaptive equipment), Bright artificial lighting, Constant noise (shared workspace), ability to rapidly switch between tasks.
