The Chief Nurse of Community Care will have oversight responsibilities for facilitating and coordinating with facilities within the Network on Community Care, clinical support and advisement on patient contract nursing home, dental program, directed care program, contract dialysis program, IV home infusion program, non-institutional care, Medical Centers Veterans Care Agreements and to support facility Community Care Program Mangers.

The major duties and responsibilities of this Nurse includes but are not limited to:
Collaborates with the VISN Deputy Network Director (DND), VISN Chief Medical Officer (CMO) , VISN Chief Nursing Officer (CNO), VISN Chief Financial Officer (CFO), The VISN Quality Management Officer(QMO), the VISN Business Implementation Manager (BIM), Facility leadership, and other stakeholders in the assessment of both facility-specific and VISN-wide program needs negatively impacting care coordination and clinical consult management. In collaboration with the DND, BIM, CFO, CNO, and CMO, assists in the development of VISN-level resource requests to address VISN-wide deficiencies. When requested and in collaboration with the DND, BIM, CFO, CNO, and CMO, assists facilities to refine written requests for additional resources (staff, space, equipment).
Incumbent utilizes and ensures all VISN 15 facilities utilize all avenues of resources available to ensure staff provide safe and efficient timely coordination of quality health care and services.
Establish, promote and monitor seamless care for veteran patients. Ensures that Medical Centers perform clinical reviews of designated VA Community care/choice consults to assure clinical documentation supports referral requests to prevent over utilization, underutilization and inefficient utilization of resources.
Administratively responsible for the clinical programming and operations of the program, as well as for the management of data related to the program.
Serves as a VISN authority in the development and implementation of projects both VISN and national developed.
Ensures that the mission and vision of the VISN15 strategic direction accords with that of VHA Office of Community Care.
Contributes significantly to VISN policy and decision-making processes. Coordinates and reviews VISN policies pertaining to the provision of program assessments and support.
Enhances customer service by allowing VISN/Facility staff to directly manage Veteran touchpoints for customer service while TPAs support staff and community provider inquiries.
Current working knowledge of assessment, diagnosis, treatment, and outcome measures of healthcare problems key to the development and deployment of Community Care programs
Academically and clinically trained to evaluate the spectrum of physical and / or mental health care services at the expert level.
Develops and implements information systems to assess and track program activities and outcomes.
Develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
Enhances customer service by allowing VISN/Facility staff to directly manage Veteran touchpoints for customer service while TPAs support staff and community provider inquiries.
Incumbent improves accountability because performance will be measured at the local Veterans Affairs Medical Center (VAMC) level to help ensure TPAs build provider networks that meet the needs of Veterans and VAMCs.
Function in support of caregivers, Veterans, and family members.
Serve as a subject matter expert and should possess advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state, and private sector resources.
Ensure the community care programs within VISN 15 service coordinates care options meet appropriate utilization of inpatient and outpatient services and communicate effectively with veterans, families, providers and outside resources.
Excellent administrative ability to prioritize and track issues, tasks, statistical data and utilization review related duties in a self-directed manner.
Work Schedule: Monday through Friday 7:30 am CST - 4:00 pm CST
Telework: Available - as determined by the agency policy. At management discretion.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Master's or Doctoral Degree from a program accredited by the Accreditation Commission for Education in Nursing or the Commission (ACEN-Formerly NCLAC) or the Collegiate Nursing Education (CCNE).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience and Certifications
National Certifications: Nurse Executive Board Certified (NE-BC), Nurse Executive Advanced Board Certified (NEA-BA), Certified Professional Healthcare Management (CPHM), Care Coordination and Transition Management (CCTM), Care Manager Certified (CMC), Accredited Case Manager (ACM), Ambulatory Care Board Certified (AMB-BC)
Type and Years of Nursing or Other Experience: 3-5 years Community Care Care Experience. 3-5 years VA Primary Care or Specialty Care Outpatient Clinic Experience
Type and Years of Leadership or Supervisory Experience: > 2 years Chief Nurse Community Care or National Office of Community Care Program Management, >3 years progressive supervisory experience
Knowledge Skills Abilities (KSA):
Provider Productivity, Community Care Program Management, Care Coordination, and Integrated Case Management, Population Health Management, Referral Coordination Initiative (RCI), Grid Management, Consult Management, VA Eligibility Standards, Open Slot Management, Provider Service Agreement. Experience with Specific Software or Programs: CPRS. Optum/Tri-West Portal. ; Scheduling Software, Veteran Self Scheduling, Consult Tool Box, Microsoft Office Products, Power-Bi, V-Signals, SHEP-Community Care, VA Health Chat, My-health-e-vet, Veteran Health Information Exchange (VHIE) Experience with Specific Groups or Organizations: Experience with Optum/Tri-West. Experience with Office of Community Care. Experience with External Accreditation (OIG & TJC).
Education Experience: Doctoral Degree Preferred.
Green or Black Belt System Redesign Experience Preferred.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse IV - Master's degree in nursing or related field with BSN and approximately 4-5 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and appropriate basic or advanced certification desired, and approximately 3-4 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: This position requires potentially long periods of continued walking, standing, and sitting. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. in the performance of duties, the incumbent may be required to drive and/or ride in a GSA-vehicles.
