The Pulmonary RN Care Manager is expected to execute position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care. The Nurse demonstrates Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning. Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes.

Duties

The Pulmonary RN is a member of Specialty Medicine Service whose role is to coordinate care of the who are scheduled for diagnostic tests, medical evaluations, and Pulmonary procedures. Non-OR procedures may be done in specialty examination rooms or procedure rooms. Care is provided to patients with the goal of achieving quality patient care by improving clinical outcomes, increasing continuity of care, and minimizing health care costs. The Pulmonary RN functions within a nursing process framework for problem-solving and care facilitation: assessment, planning, implementation, and evaluation. Job duties are focused on achieving optimum mental and physical health, and illness prevention. S/he is expected to perform data collection and analysis for the purpose of improving care to patient groups, maximizing utilization of resources. Nursing practice includes an emphasis on health promotion, accountability, assessment, education, infection control, safety, psychological support, administration, and research. A multidisciplinary approach is used to assist the patient, family and/or significant other(s) to attain optimal functioning ability Nursing care includes but are not limited to the following:: Assessment, planning, intervention, and evaluation of patients navigating through all types of Pulmonary office visits. Coordination and management of patient care during all phases of diagnostic tests and procedures, treatments, and discharge phase of care. Implementation of standards of care and practice for patients undergoing diagnostic procedures Implementation of procedural education/prepping, specific positioning, and intra-procedural care. Interpretation of signs, symptoms, and diagnostic monitoring with reporting documentation, and implementation of ordered treatments. Operation of specialized equipment associated with care, diagnosis or treatment of cardiology patients. S/he is responsible for demonstrating and maintaining full competency in the use and pre-treatment of reusable medical equipment (RME). S/he must ensure that the appropriate Standard Operating Procedure (SOP) and manufacturer's instructions are followed for any step in the use of RME treatment in which s/he is involved. The Pulmonary RN is responsible for working in close collaboration with providers. S/he possesses the ability to provide a comprehensive physical assessment and triage patients in direct encounters and by telephone in response to the patient's expressed needs. Care hand-off is communicated using SBAR (Situation/Background/Assessment/Recommendations) communication technique. Schedules patients into clinic appointments. Collaborates with other VA ACC services for information necessary for patient's care. May assist with patient examinations and be present during patient visits for initiation of contact and coordination for patients in endocrinology and rheumatology clinics as needed. Coordinates completion of all ordered tests and secures results prior to patient visit with provider. Maintains continuous contact with patient/family regarding scheduled tests, and follow-up care, or changes in the treatment plan. Facilitates communications between departments, and other providers as indicated, including scheduling focused meetings to discuss care and/or treatment for complex patients. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement,09/07/2023. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant. [NOTE: See exception in subparagraph (b) below for candidates who are enrolled in a MSN Bridge Program.] The Accreditation Commission for Education in Nursing (ACEN), previously known as the National League for Nursing Accrediting Commission (NLNAC) OR The Commission on Collegiate Nursing Education (CCNE). The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. In these programs, students are given a certificate of professional nursing to sit for the NCLEX and should submit this certification to VA prior to appointment. Students do not earn a BSN, but receive a MSN degree upon completion of course work. A copy of the MSN transcript must be provided to VA. Bridge programs that confer a master's degree also fully meet the education requirement, even though a Bachelor's Degree is not awarded. OR In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Registration: A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The appointing official may waive this registration if the RN is to serve in a country other than the U.S. and the RN has registration in that country (e.g., Philippines). The RN must maintain a current, full, active and unrestricted registration to continue employment with VA. English Language Proficiency: RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR An ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR A Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR An ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR A BSN with approximately 1-2 years of nursing practice/experience; OR A Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR A Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR A Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR A Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Physical Requirements: This position requires potentially long periods of continued walking, standing, and sitting. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing. S/he may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. S/he must be a mature, flexible, sensible individual capable of working effectively in stressful situations, and able to shift priorities based on patient needs. Applicants must possess the mental and physical ability to satisfactorily perform the duties of the proposed assignment. Physical requirements may involve lifting, carrying, pulling, pushing, reaching above shoulder, walking, standing, sitting, kneeling, and repeated bending. S/he must complete annual Employee Health requirements, such as TB screening or testing on hire, as a condition of employment.
