The VA Finger Lakes Health Care System is currently recruiting for one Social Worker (Program Coordinator) for the Psychosocial Rehabilitation and Recovery System Center (PRRC) and Mental Health Intensive Case Management (MHICM) Programs. The incumbent is responsible for the professional, technical, and administrative functions of the PRRC and MHICM programs and provides clinical services such as individual therapy, group and family counseling, and clinical assessments.

Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. Performs program analysis, development, review, design. Responsible for establishing and maintaining working relationships within and between departments within the VA and with agencies and providers external to the VA. Performs work related to the review, monitoring, analysis, planning and assessment of various programs. Responsible for key project activities related to the planning, development, and implementation of the project component. Patient Care Independently performs case finding functions and outreach, identifies and screens patients for psychosocial needs. Independently conducts psychosocial assessments. Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team. Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Independently identifies high-risk patients and provides case management services. Provides crisis intervention services, addressing the cause as well as the presenting complaint. Coordinates family conferences and serves as liaison to family members. Establishes and maintains effective therapeutic relationships with veterans and their families. Works independently with patients and families experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills. Independently provides consultation services to other staff members regarding the psychosocial needs of veterans. Provides consultation on the impact of psychosocial problems on health care and adherence to treatment plans. Evaluates own practice through professional peer review, case conferences and other organizational means. Communicates effectively, both orally and in writing, with people from varied backgrounds. Possesses general knowledge of veterans' benefits and services relating to special programs to include service connected compensation and non-service connected pension. Facilitates referrals based on veterans' needs and eligibility. Meets the needs of customers while still supporting VA missions. Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful and respectful manner. Provides the customer with consistent information according to established policies and procedures. Handles conflict and problems in dealing with the customer constructively and appropriately. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).
