As the VISN 19 Group Practice Manager and Community Care Coordination Program Manager, the incumbent develops innovative approaches to improve outpatient access to care and community care coordination in VISN 19 facilities.
Duties
The VISN 19 Group Practice Manager-Community Care Coordination Program Manager's primary role is to provide oversight of access to Direct Care (VA services) and to Care in the Community and focuses on the balance/relationship between these services and the associated integrated care coordination processes. This position collaborates with facility Group Practice Managers and Community Care managers at the 8 VAMCs and associated outpatient clinics throughout VISN 19 on issues related to access to care. The incumbent will have responsibilities that include project management, programmatic oversight, data analysis, strategic and business planning, and performance improvement. This position reports to the VISN 19 Chief Medical Officer or designee within the CMO Office.
As the VISN 19 Group Practice Manager-Community Care Coordination Program Manager, the incumbent develops innovative approaches to improve outpatient access to care and community care coordination in VISN 19 facilities.
Uses quality assessment, performance improvement, staff education and development, planning, and resource management to accomplish goals.
Reviews, analyzes, and provides recommendations regarding clinic management to ensure VISN compliance with VHA Handbooks, Directives, Manuals, and Memorandums with particular focus on community care eligibility and care coordination processes.
Plans and evaluates assigned programs, leads, and/or participates on committees and workgroups at the VISN levels works on special projects, identifies opportunities to improve access to care in VA and in the community, and facilitates the development of creative and innovative business practices.
Provides coordination and direction to achieve optimal utilization of available resources while breaking down barriers in work systems and capitalizing on the synergy of all VISN 19 facilities.
Oversees VHA initiatives focused on care coordination such as the Referral Coordination Initiative
Works with facilities and VISN business office staff to implement community care coordination program requirements.
Guides the full implementation of VA Clinic Practice Management and community care coordination.
Responsible for anticipating and/or identifying problems which require revisions to care coordination procedures to ensure program effectiveness.
Performs comprehensive analysis on the quality, efficiency, effectiveness, and utilization of resources to ensure workload and productivity are both balanced and maximized. This analysis includes the use of numerous VSSC databases to identify trends for use in program planning.
Conducts focused program reviews at the request of senior VISN and Facility leadership.
Works with facility GPMs to develop proposals, plans of action, and recommendations ensuring that clinical programs are effectively providing timely access to high quality, coordinated care in both direct care and community care services.
Must apply specialized principles and practices of health care management in directing and/or supporting a complex and varied healthcare delivery system.
Works with GPMs to integrate data analytics and measures with continuous process improvement utilizing LEAN, Six Sigma, and other improvement methodologies to meet and sustain goals and objectives.
Serve as an internal consultant to VISN GPMs and Community Care staff on specific quality and process improvement and organizational development projects such as aligning organizational processes and reinforcing new practices.
Assists in training facility GPMs in clinic practice management concepts, care coordination, make-buy analyses.
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.
Work Schedule: Monday-Friday, 7:30am-4:00pm

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 09/06/2023.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-12 position you must have served 52 weeks at the GS-11. For a GS-13 position you must have served 52 weeks at the GS-12. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Individual Occupational Requirements:
Undergraduate and Graduate Education: Major study -- hospital administration, public health administration, or related fields such as business or public administration with course work in health care administration.
OR
Specialized Experience:
GS-12: You must have one year of specialized experience equivalent to at least the next lower grade GS-11 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include assisting in collaborating with business office staff to develop and implement a team-based care coordination model for referral and delivery of community care; assisting in providing staff assistance to VISN leadership including special studies, surveys, projects, planning, statistical analyses and general staff support. This work may have been performed in an operating health care facility or a higher organizational echelon with advisory or directional authority over such facilities. Work must have involved a close working relationship with facility managers and analysis and/or coordination of administrative, clinical, or other service activities, and provided knowledge of the following:
Missions, organizations, programs, and requirements of health care delivery systems;
Regulations and standards of various regulatory and credentialing groups; and
Government-wide, agency, and facility systems and requirements in various administrative areas such as budget, personnel, and procurement.
GS-13: You must have one year of specialized experience equivalent to at least the next lower grade GS-12 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include serving as subject matter expert and coach to facility Group Practice Managers to ensure consistency in program deployment and clinic management principles; collaborating with business office staff to develop and implement a team based care coordination model for referral and delivery of community care; providing staff assistance to VISN leadership including special studies, surveys, projects, planning, statistical analyses and general staff support This work may have been performed in an operating health care facility or a higher organizational echelon with advisory or directional authority over such facilities. Work must have involved a close working relationship with facility managers and analysis and/or coordination of administrative, clinical, or other service activities, and provided knowledge of the following:
Missions, organizations, programs, and requirements of health care delivery systems;

Physical Requirements: No special physical exertion is required. The work is primarily performed while sitting though some work may require periods of standing. The regular and recurring work of the position involves sitting at a desk, conferences, meetings, etc., and occasional visits to activity work sites. Occasionally incumbent is required to carry light objects, such as files, mail, supplies and papers.
