This position is located within Mental Health Service at Eastern Oklahoma VA Healthcare System (EOVAHCS) and serves as the Primary Care Management Module Coordinator for Mental Health Services at EOVHACS, including the associated Community Outpatient Clinics (CBOCs). The incumbent will help with training, implementation, monitoring and evaluation of all administrative procedures related to MH Patient Care Management Module (PCMM).
Duties
Completes reports for health care team; manage inactivation list to include but not limited to deaths, potential inactivated patients, transfers, and dual assignments.
Analyzes and evaluates, on a quantitative and qualitative basis, the efficiency and effectiveness of the PCMM database for all Mental Health Services throughout the hospital service area.
Educates and trains providers, administrative, and clinical staff on PCMM.
Collaborates with the Facility and VISN Clinical Applications Coordinators (CACs) as well as IRM staff to produce monthly reports regarding provider specific quality measures displaying historical, cumulative, and comparative data.
Determines potential problem areas through workflow analysis and proceed to make recommendation these areas to correct or improve the PCMM program through efficient coordination between various providers, teams and clinics as well as recommending new ways of eliminating, combining, simplifying, or improving procedures and processes.
Prepares and define teams; assign staff to positions on defined teams; assign patients to teams; assign patients to practitioner via team position and enroll in clinic.
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.
Work Schedule: Monday - Friday, 8:00am - 4:30pm

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/29/2023.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05. For GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.

GS-07 Grade Determination:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: utilizing provider panels for scheduling patient appointments; reviewing clinical reports for consult management and follow-up; applying medical center policies and procedures affecting patient assignment; advising on healthcare delivery, the mission of the VA, medical center policy, procedures, and VHA regulations as they relate to different clinical functions; reporting deceased patients, utilizing healthcare computer system for data entry and integrity; operating a personal computer; utilizing numerous software applications including Microsoft Office Suite, graphic displays for reporting data; communicating both written and orally; preparing memorandas, letters, procedures, and reports; and applying standardized administrative practices and procedures. OR,
Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have one (1) full year of graduate level education or superior academic achievement. OR,
Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond graduate level (in excess of 1 year of full-time study or 18 hours).
GS-09 Grade Determination:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to adopting the mission of VHA, Medical Center policies, procedures, VHA regulations as they relate to many different clinical functions, and healthcare delivery; applying clinical practices as they relate to PCMM and clinic creation and maintenance; interpreting Medical Center policies/procedures affecting place of assignment; understanding of the PCMM program, data integrity, PCMM assign/unassignment, removal of deceased patients, monitoring provider panel size, etc.; utilizing the Medical Center's computer system (VISTA) for entry and retrieval of administrative and clinical data, to include numerous existing report options and specialized queries, requiring Fileman extracts; utilizing a wide variety of software to include mental health PCMM software, CPRS, VISTA, Excel, etc. and understanding overall impact that mental health PCMM reports have on medical center decision, medical center's overall system of providing care to Veterans, medical centers and VAMC's organizational system; understanding of statistical process control and computer programs that display graphic displays of data to identify sources of variability; communicating effectively, both written and oral, that reflect a compassionate, competent, and professional response to the needs of both internal and external clients; evaluating the clerical work processes to identify, analyze, and recommend solutions to problems in either organizational structure, staffing, administrative procedures, work processes, or workload distribution; assisting staff and ensuring performance measures are being met to ensure compliance to the facility, the VISN and national guidelines, memorandums and/or directives. OR,

To qualify based on education for this grade level you must have a master's or equivalent graduate degree or 2 full years of progressively higher level graduate education leading to such a degree or LL.B. or J.D., if related. Physical Requirements: Work is sometimes sedentary but requires walking, standing and carrying of medium weight items, such as, patient records. Incumbent works extensively in the hospital computer system. Incumbent must have an even temperament and act in a professional manner during work performance.
Work Environment: The employee works with team members as well as various other facility personnel. The employee may encounter patients, with which they are assigning and should be informed of how to handle such patients if the need arises (i.e. Suicide Prevention Techniques, Psychiatric Emergency Room contacts, etc.) The work area is adequately lighted, heated, and ventilated.

Superior Academic Achievement (SAA): Applicants who wish to apply under this provision must submit proof from their educational institution that they meet one of the criteria listed below.
