GS-09: A housing subsidy of $483.00 without dependents and $573.00 with dependents may be available. GS-11: A housing subsidy of $618.00 without dependents and $720.00 with dependents may be available. This position is located at the Northern Cheyenne Service Unit in Lame Deer, MT. The purpose of this position is to provide specialized case management/care coordination for patients requiring special health care needs. The incumbent reports to the Director of Nursing or designee.

Duties

Reviews all IHS patient referrals to assist with determination/verification of patient eligibility for Direct, PRC (Purchase Referred Care), and alternate resources for the coordination of referred services. Works in collaboration with the Purchase Referred Care Services Department to manage referred patients to improve coordination of services; provide resources for alternate care, to improve access to care and ensure appropriate utilization of he entire spectrum of services and to contain and/or decrease costs. Maintains tracking system to monitor patients referrals throughout treatment including patients with chronic illnesses that require frequent reassessment and care planning to coordinate care with outlying healthcare facilities' to decrease fragmentation of care for improved quality of life and cost containment. Must maintain ongoing open communication with patients and families while maintaining confidentiality in accordance with HIPAA laws, departmental policy and professional standards. Responsible for providing ongoing education and assistance to primary care teams regarding rules and regulations of PRC eligibility and justification in patient referral processes. Responsible for maintaining adequate inventory of direct patient care supplies through P-Card management and UFMS requestor. Responsible for reporting supply budget status to DON on a monthly basis. Responsible for identifying capital equipment needs, collaborates with FMA and DON in procurement of capital equipment. This position will support the Federal Government's response efforts to COVID-19.

Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. Individual Occupational Requirements NOTE: Employees currently assigned to positions in this occupational series as of September 2017 will be considered to have met the basic requirements for the position occupied. BASIC REQUIREMENTS Education A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing. Basic Requirements for the GS-9 (or equivalent) Grade Level In addition to the above requirements, applicants must meet one of the following requirements: Possession of a master's degree in professional nursing; or At least one full year of professional nursing experience (equivalent to the GS-7 grade level or above) and possession of a diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program. Basic Requirements for the GS-11 (or equivalent) Grade Level In addition to the above requirements, applicants must meet one of the following requirements: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: GS-9: Provide basic health care teaching, crisis intervention, stabilization, and direct counseling services to persons in crisis as individual needs arise; perform intake screening and evaluation; maintain health records. GS-11: Develop a tracking system for patient care coordination and care management. Collaborate with physicians, plan and provide nursing care applying nursing practices and medical protocols. Provide case management for patients by coordinating services to meet patient's care goals. You must meet all qualification requirements within 30 days of the closing date of the announcement.
