This position is within the Indian Health Service, Outpatient Department at the Northern Cheyenne Service Unit, Lame Deer, MT. The purpose of this position is to provide nursing care coordination to a primary care panel of patients. This position reports to the Director of Nursing.
Duties
Ensure Patients are routed to appropriate service, assisting patient with health literacy and serving as their advocate.
Responsible for coordinating care with other departments and programs developing and implementing patient care and education activities necessary to reach positive patient outcomes.
Follow up with empaneled patients to ensure appointments are kept, improve adequacy of young adult and adult patient health and tracking patients seen by various Specialty providers.
Plans and participates in Programs for Primary Care Medical Home (PCMH) Management and Primary Prevention Services and tracks patient data outcomes.

BASIC REQUIREMENT(S):
Education
A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure
For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience
The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing.
Basic Requirements for the GS-10 (or equivalent) Grade Level
In addition to the above requirements, applicants must meet one of the following requirements: At least one full year of professional nursing experience (equivalent to the GS-9 grade level or above) and possession of a diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program.
In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below.
MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Promote self-management of chronic disease through helping patients to identify barriers to making healthful choices. Help patient identify barriers to adhere to the prescribed regimen, providing coaching and guidance on positive health behaviors and providing chronic disease management. Assess, plan, implement and evaluate the care of patients with medical, surgical, pediatric, newborn, obstetric, ambulatory and emergency conditions in the primary care clinic. Provide chart review to identify patients at risk for increased morbidity and mortality and recommended interventions. Provide patient and family education to assist in understanding and accept implications of care, diagnosis and treatment.
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
