This position is responsible for the delivery of Whole Health Coaching and skill development of the local Whole Health Coach program at the VA Medical Center in San Antonio, TX.
Duties

Contacts eligible participants for enrollment and explain the health coaching process;
Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones;
Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings;
Conducts Whole Health Facilitated Groups;
Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments;
Guides Veterans in the exploration of core elements of personal health planning through individual and group-based programs;
Encourages and assists Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran's goals;
Identifies habits and behaviors that are harmful to quality of life and related to chronic illness;
Develops treatment plans for Veterans focused on behavioral interventions based on current scientific knowledge and research that improve quality of life and reduce risk for chronic disease, or that reverse or slow progression of chronic disease;
Reviews current health behaviors and develops customized treatment plans with Veterans;
Individualizes intervention strategies to help the client utilize existing skills and develop new ones to enhance success;
Schedules ongoing sessions to assess progress and assist in navigating obstacles or challenges;
Guides Veterans to learn about available support tools in the VA and the community that assist in attainment of goals;
Coordinates care with Veterans' treatment providers, both primary and specialty care, to ensure identified goals are consistent with best practices for treatment and management of chronic illness;
Utilizes motivational interviewing and health coaching techniques and processes, learned through pre-requisite health coach training, to assist members in developing a realistic plan to accomplish their goals;
Fosters an environment of individual responsibility to maximize each Veteran's ability to adhere to a lifestyle management plan;
Develops trust with Veterans and incorporates trust principles in day-to-day activities;
Works collaboratively within a team setting to provide positive outcomes for participants, peers and overall operations;
Maintains education and knowledge regarding lifestyle (habits and behaviors) and clinical outcomes, including behaviors that improve Veterans' health and well-being and those related to poor clinical outcomes;
Coordinates with others on the Veteran's health care team to ensure optimal patient care;
Provides eligible Veterans and family members with information regarding lifestyle management including weight management, nutrition, exercise/fitness, smoking cessation, stress management, diabetes, pain management, and other appropriate conditions that are in line with the personalized health plan developed between the health care team and the patient;
Uses VISTA, CPRS, Microsoft office software, Intranet and Internet and other applicable software to track and support patient progress;
Communicates proactively and effectively with Veterans, peers, and supervisors;
Develops information for Veterans about the Whole Health Group and Individual Programs;
Conducts outreach activities to inform Veterans and family members of available programs;
Provides educational opportunities for staff to promote health coaching and encourage referrals to these services;
Develops and conducts onsite educational series to explain health coaching, including its uses and benefits;
Participates in health fairs in the community;
Assists with data management analyses, special studies, and reports for use by management for budget preparation, workload projections, and quality assurance and utilization reviews;
Participates in the establishment of administrative standards to improve programs, facilities, and operations;
Provides training and troubleshooting for other health coaches in health coaching administrative processes and associated supporting issues;
Performing related duties as required.
Work Schedule: Monday through Friday, 8:00am - 4:30pm (subject to change).

Qualifications
Individual Occupational Requirements: Basic Requirements
Possess a Degree: behavioral or social science; or related disciplines appropriate to the position.(UNOFFICIAL TRANSCRIPTS REQUIRED).
OR
Possess a combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.(UNOFFICIAL TRANSCRIPTS REQUIRED).
OR
Possess four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.
AND
In addition to possessing the basic requirements above, you must also meet the minimum qualifications below to qualify based on your experience and/or education described below.
For GS-7:
Possess a bachelor's degree in a field that equips me to perform the work of the position, and I have Superior Academic Achievement (S.A.A.). S.A.A. is based on (1) class standing, (2) grade-point average, or (3) honor society membership. (UNOFFICIAL TRANSCRIPTS REQUIRED).
(1.) Class standing -- Applicants must be in the upper third of the graduating class in the college, university, or major subdivision, such as the College of Liberal Arts or the School of Business Administration, based on completed courses.
(2.) Grade-point average (G.P.A.)-- Applicants must have a grade-point average of: (a) 3.0 or higher out of a possible 4.0 ("B" or better) as recorded on their official transcript, or as computed based on 4 years of education, or as computed based on courses completed during the final 2 years of the curriculum; or (b) 3.5 or higher out of a possible 4.0 ("B+" or better) based on the average of the required courses completed in the major field or the required courses in the major field completed during the final 2 years of the curriculum. Grade-point averages are to be rounded to one decimal place. For example, 2.95 will round to 3.0 and 2.94 will round to 2.9.
(3.) Election to membership in a national scholastic honor society -- Applicants can be considered eligible based on membership in one of the national scholastic honor societies listed below. These honor societies are listed by the Association of College Honor Societies . Agencies considering eligibility based on any society not included in the following list must ensure that the honor society meets the minimum requirements of the Association of College Honor Societies. Membership in a freshman honor society cannot be used to meet the requirements of this provision.
OR
Possess at least one full year of graduate level study or possess a master's or Ph.D. in a field that provided the knowledge, skills, and abilities necessary to do the work. Such fields include the social or behavioral sciences. (UNOFFICIAL TRANSCRIPTS REQUIRED).
OR
Possess at least one year of specialized experience equivalent to the GS-5 grade level in federal service. Specialized experience includes developing treatment plans for patients focused on behavioral interventions based on current scientific knowledge and/or research that improve quality of life and reduce risk for chronic disease, or that reverse or slow progression of chronic disease; possessing knowledge regarding lifestyle (habits and behaviors) and clinical outcomes, or behaviors that improve patients health and well-being and those related to poor clinical outcomes; or working with patients on identifying motivation for change, determining goals for their work together, implementing treatment plans, or assessing progress in order to adjust plans as necessary.
OR
Possess a combination of education and experience listed above for GS-7. To calculate the combination of education and experience, determine the amount of experience you have as a percentage of the experience required for the grade level. Then determine the amount of education you have a percentage of the education required for the grade level. Add the two percentages; the total percentage must equal at least 100% to qualify for the grade level of GS-7. Note: A transcript or course listing must be submitted if you are basing all or part of your qualifications on education. (UNOFFICIAL TRANSCRIPTS REQUIRED).
