Incumbent serves as program coordinator at the GS-12 grade and is administratively responsible for the Homemaker/Home Health Aide and Respite programs. She/he provides direct patient care services in the program area. Incumbent manages the daily operation of the programs, develops policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use.

The incumbent manages between several agency agreements. Specific tasks include: Conducting visits to assess and determine eligibility, quarterly reviews of agency care plans and medical documentation, semi-annual review of agency supervisory reports, annual onsite audit of agency for compliance, review of billing/invoices to Community Care, and prepares regular reports. Respite Program: As the Respite Program Coordinator, the Social Worker assists in providing services to veterans who would, without such in-home services, require nursing home level of care. The incumbent manages between several agency agreements. Assists Veterans to obtain in-home respite services within national directive guidelines for those eligible. Major Duties: Specific tasks include: assess and determine eligibility, reviews of agency care plans and medical documentation, review of billing/invoices to Community Care, and prepares regular reports. The Social Worker independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. The incumbent must possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups. The incumbent independently conducts psychosocial assessments. He/She develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team. The incumbent possesses the skills to offer psychosocial interventions and measures outcomes to interventions. The Social Worker is able to coordinate community-based services, including information and referral for additional services from other VA, government, and community agency programs. The incumbent independently identifies high-risk patients and provides case management services. He/She provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences, and serves as liaison to family members. The Social Worker has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures; this includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. The Social Worker establishes and maintains effective therapeutic relationships with veterans and their families. She/He is able to independently work with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group and family counseling and therapy skills. The incumbent independently provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan. He/She has the ability to evaluate his/her own practice through professional peer review, case conference, and other organizational means. The incumbent is able to communicate effectively, both orally and in writing, with people from varied backgrounds. General knowledge of veterans' benefits and services relating to special programs, service connected compensation, and non-service connected pension is required. He/She facilitates referrals based upon veterans needs and eligibility. The Social Worker performs other related duties as assigned. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Senior Social Worker 0185, GS-12 Experience/Education: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. The actual grade at which an applicant may be selected for this vacancy is GS-12. Physical Requirements: This work is primarily sedentary but also requires some standing, walking, bending, lifting and carrying of items.
