This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The Clinical Care Coordinator ensures the patient's efficient movement through the healthcare continuum to included discharge planning and/or case management services. Ensures continuity of care from the time of admission until discharge with post discharge follow-up systems and linkages when appropriate. Is accountable for reviewing/monitoring the appropriateness and necessity of admissions, intensity of service, and continued stay. Provides patient/family education and coordinates the Interdisciplinary Discharge Planning Conferences. Leads physicians, nurses, case managers, utilization management personnel, other social workers, outside agencies, and ancillary departments in developing a discharge care plan and/or case management care plan that effectively manage clinical and financial outcomes. Identifies opportunities to reduce costs and length of stay while maintaining or enhancing the quality of patient care and services. The Clinical Care Coordinator is responsible for clinical and administrative duties: a) Monitors and manages incoming actions from initial receipt to completion, b) Assessing multi-factorial medical, emotional and psychosocial needs as they relate to or contribute to the patient complaint and creating a plan as needed to adequately respond to patient's needs, serving as an advocate for the patient and family c) Assisting patients in understanding their rights and responsibilities as Veterans receiving VA health care services. d) Receiving and listening to complaints and grievances from patients and family members unresolved at the service level. Develop strategies to assist resolution at the service level e) Effectively utilizing a variety of tools to monitor and document patient satisfaction including but not limited to Patient Advocate Tracking System (PATS), LEAF, and SWS internal tracking. f) Ensuring entry of all Reports of Contact (ROC); congressional inquiries; clinical appeals and final decisions into the LEAF and Patient Advocate Tracking System as indicated by VHA Policy. g) Understanding facility and VHA-wide directives, laws and other governances which apply to patient rights and responsibilities. h) Identifying needs for social work assessment, evaluation, and treatment and providing recommendations to Supervisory Clinical Care Coordinator. i) Reporting and documenting potentially threatening behavior to appropriate Authorities in accordance with VHA regulations. j) Working with Clinical Care Coordinator and Social Work Executive to understand the training and orientation needs of the social workers. k) The Clinical Care Coordinator should be knowledgeable about the patient's rights, the patient's wishes and the administrative process involved. The Social Worker makes numerous daily contacts with Veterans, families, physicians, community agencies and other staff members to obtain and relay information on Veterans' problems. This unique ombudsman position is designed to channel patient related complaints, problems and inquiries with the expertise and authority to obtain hospital-wide resolutions. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure-Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ? 7403(f). GS 11 Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. NOTE: For appointment licensure or certification at this level please refer to paragraph 3c. OR Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Preferred Experience: Excellent oral and written communication skills and ability to problem solve. The full performance level of this vacancy is 11.
