Incumbent serves as the Fayetteville North Carolina VA Coastal Health Care System's Care Coordination & Integrated Case Management (CC&ICM) Social Work Coordinator at the GS-12 grade The GS-12 Coordinator has expertise in coordinated care and serves as the integration liaison, alongside the CC&ICM Co-Champion for Nursing Service, between Executive Leadership and facility staff.

As a CC&ICM Co-Champion, serves as the integration liaison among the Service Chief, Executive Leadership, and staff. Facilitates staff access to CM trainings and education. Facilitates and coordinates trainings on nationally recognized CM standards of practice to ensure delivery of professional CM services across the health care continuum.
Ensures case management services are being delivered in accordance with VHA Directive 1110.04 Integrated Case Management Standards of Practice.
Oversees the Care Coordination Review Team (CCRT), an inter-professional and inter-departmental team comprised of specialty CM programs, primary care, and mental health staff with experience in care coordination that conducts high-level reviews of cases needing special attention. Makes LC assignments in partnership with the Veterans. Facilitates matching the Veteran's acuity and complexity with the type and intensity of the intervention(s). Leads, tracks, and monitors workload, documentation, and coding requirements for case management, including reporting to leadership and consulting with CM supervisors. Completes, maintains, revises, and reports on Implementation and Education plans to both local leadership and national CC&ICM program office.
Serves as a Lead Coordinator, a single, readily accessible, and clearly identifiable point of contact for a Veteran, his/her family and caregiver(s), and care team members. Ensures Veteran care is coordinated across settings, services, and episodes of care, and the care plan is delivered as clinically indicated. Oversees care coordination and facilitates inter-professional team communications, reducing task and intervention duplication and improving the quality-of-care plan delivery. Engages Veterans in Self-Management and Shared Decision Making.
Serves as a consultant and subject matter expert on CM practice, including keeping abreast of VA policies, laws, and regulations that affect CM delivery to Veterans, their families, and caregivers. Coordinates and collaborates with the VA medical facility level Sponsor(s), care coordination, care management, and CM service area and program leads. Collaborates with internal stakeholders to assess CM composition, structure, and services to identify promising practices and gaps in CM services and identifying opportunities to enhance communication, collaboration, and coordination across the care continuum.
Facilitates the completion of a Readiness Assessment for the purpose of identifying VA medical facility strengths and gaps in CM services. Ensures a CCRT is in place to facilitate safe transitions of care from one level of care coordination or setting to another and with the appropriate assignment of staff as LC. Establishes collaborative relationships with all clinical disciplines and all support staff to foster a culture wherein care coordination is the responsibility of all staff.
Identifies and monitors CM metrics in collaboration with health informatics and prepares program and performance metric reports for VA medical facility leadership on a quarterly basis, including leading development of a local CC&ICM dashboard and coordinating metrics gathered locally with national data collection and reporting.
Identifies community resources (i.e., local, State, and national) available to provide continuity of care and to enhance the quality of life for the Servicemember or Veteran and disseminates this information annually to Care Coordinators, Care Managers, and Case Managers within the VA medical facility. Establishes and maintains contact with other CC&ICM Champions to ensure sharing of best practices and promote standards of practice across VHA. Participates in multiple meetings at the VISN, consortium, and national levels regarding CC&ICM. Serves on VISN, Consortium and National Workgroups as available.
Collaborates with various services and programs throughout the Fayetteville North Carolina VA Coastal Health Care System to implement and promote CC&ICM. Leads key project activities related to the planning, development, and implementation of the program. Briefs Service, Facility, and VISN leadership as needed regarding implementation and sustainment of VHA Directive 1110.04 Integrated Case Management Standards of Practice.
Provides backup coverage for other Social Workers in the service and performs other related duties as assigned.

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ? 7403(f).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: Social Worker (Program Coordinator), GS-12 (1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Preferred Experience: 2 years post LCSW experience preferred. Also experience with complex case management, discharge planning, and knowledge of community resources.
