The Primary Care Mental Health Initiative (PCMHI) Social Worker delivers evidence based care for primary care patients identified with a variety of emotional and psychiatric problems including depression, anxiety disorders, and alcohol misuse. The primary role of this position is to serve as team Lead while providing a full range of brief, solution focused therapeutic interventions to primary care patients not currently established with a mental health provider.

Duties

The duties of the Senior Social Worker in the Primary Care Mental Health Integrated Care Clinic include, but are not limited to: *Provides direct services for the Veteran within the Primary Care Service Line that currently are new to the VAMC or who do not have a mental health provider. *Provides training for other members of a multidisciplinary treatment team in areas related to mental health assessment, treatment, and behavior management, life skills, and co-occurring disorders. Independently conducts intake and needs assessment of Veterans that present to PACT for services related to a mood disorder. Identifies symptoms leading to clinical diagnoses of a mental health disorder. *Provides all necessary assessment functions, e.g. interviewing, psychosocial histories, and mental health assessments to aid in the development of treatment plans and clinical assignments. Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. *Must possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups to the extent that they are able to serve as the model for such care within their teams. Psychotherapy will be evidence-based in nature whenever available for a particular condition, and will utilize best practices to the extent possible in all patient care interactions. Must be readily able to provide consultation, direction, and answer questions for both PACT and their respective teams in consultation and communication with PCMHI leadership and with fidelity to the PCMHI program and best clinical interests of Veterans. *Develops treatment plans with the Veteran that address the factors that contributed to mental health referral and other co-occurring factors within a recovery model framework. Works with the Veteran to realize the Veteran's treatment goals, and reassesses and revises goals as indicated by the Veteran's engagement in treatment. *Ability to manage day to day operations of Program in Coordinators absence and to conduct meetings, trainings and education for new staff. *Provides Social Work support to the PC/MHI and the PACT Teams as a functional member of both teams, to include consultation and interventions. Serve as a member of a multidisciplinary team, and shall provide timely and succinct feedback to primary care providers regarding consultation findings and recommendations. *Provide concise documentation of care and recommendations in patient's medical record and will determine the appropriateness of the patient receiving services in the primary care setting. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. (TRANSCRIPTS REQUIRED) Licensed or certified Must have current License Clinical Social Work (LCSW). Persons hired or reassigned to Social Worker positions in the GS-0185 series in VHA must be licensed by a state to independently practice Social Work at the Advanced Master's Degree level. (LICENSE REQUIRED) Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. GS-12 (Above Full Performance) To qualify for the Senior Social Worker Position at the GS-12 level, the following experience must be demonstrated on your resume. Education may be substituted for the experience at the GS-12 level. Experience/Education: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be License Clinical Social Work (LCSW) licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Senior Social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: PCMHI (The Primary Care Mental Health Initiative) experience is preferred but not necessary. Advance performance level requires completion of a minimum of 3 years of post-MSW degree experience in the field of health care social work with an emphasis on mental health (VA or non-VA experience) and licensure or certification in a state at the independent practice level. The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is above the full performance level at a GS-12.
