The Cheyenne VA Health Care System is looking for one Senior Social Worker to join the Home Based Primary Care Social Work Service. Home Based Primary Care (HBPC) is a VA home care program in the Geriatric Extended Care (GEC) spectrum that provides comprehensive, interdisciplinary primary care in the homes of Veterans with serious medical, social, and behavioral conditions for whom routine clinic-based care is not effective or many not be possible.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The Home Based Primary Care (HBPC) Senior Social Worker, is expected to provide high quality clinical care and case management, to a panel of no more than 100 Veterans with complex and specialized needs who are enrolled in the HBPC Special Population PACT program at CVAHCS. HBPC is a VA home care program in the Geriatric Extended Care (GEC) spectrum that provides comprehensive, interdisciplinary primary care in the homes of Veterans with serious medical, social, and behavioral conditions for whom routine clinic-based care is not effective or many not be possible. HBPC serves Veterans as they face the challenges of disability, aging, and chronic disease by providing comprehensive longitudinal home care which promotes independence, wellness, and quality of life. The HBPC Senior Social Worker screens and assesses the Veteran and his/her caregivers' psychosocial needs and barriers in order to facilitate his or her access to appropriate VA and community resources. Through collaborative relationships with the Veteran, his or her family/caregiver and other VA programs and VA staff, The HBPC Senior Social Worker coordinates the gathering of information vital to the process of setting appropriate care goals and time frames for achieving those goals. Assessment information may include, but is not limited to: - Marital/family/social/care giver status - Medical status - Guardianship, POA, DPOA, other legal issues - Financial status - Use of entitlement programs - Strengths/Limitations - Long-term care planning - Emergency Caregiver planning Determines realistic goals with the Veteran and his/her caregiver, including such factors as a) the appropriate type of intervention and/or level of care, b) the setting conducive to providing such intervention or care, c) the need for intervention to allow for the least restrictive level of care, d) transportation options/barriers and e) the steps to obtaining resources (VA or community) that make the achievement of goals possible. Provides case management services as needed to Veterans and their caregivers. As a lead coordinator for the CCICM program, The HBPC Senior Social Worker may be assigned as a lead coordinator for Veterans receiving services through HBPC. Arranges referrals to needed VA and/or community resources (e.g., home health care, services available through area agencies on aging, community support groups, hospice care). Additionally, The HBPC Senior Social Worker will educate the Veteran and/or caregiver about the available services and their shared responsibility in accessing such services. The Veteran's preferences or that of his or her surrogate decision-maker will be considered throughout the referral and care coordination process. Monitors patterns of resource availability, identifying unmet needs and the resultant impact on the Veteran's psychosocial functioning. The HBPC Senior Social Worker will communicate with appropriate stakeholders to encourage the development of new and innovative resources. Advocates for Veterans and caregivers to ensure that they have access to all needed care and resources within VA and in the community. By availing the Veteran and caregiver of his or her choices in complex decisions and by guiding them through complicated enrollment procedures and bureaucratic processes, The HBPC Senior Social Worker facilitates the best possible outcome for the Veteran and caregiver. When appropriate and feasible, The HBPC Senior Social Worker will educate and encourage the Veteran and his or her family/caregiver to advocate on his/her own behalf, fostering independence and empowerment. Assesses and intervene in any medical or psychiatric crises presented by the Veteran and participate in developing crisis management and safety plans to minimize the event of future crises. Provides education about VA and community resources, entitlement programs, VA benefits and Advance Directives/Living, completing the advanced directive or living will as requested. The HBPC Senior Social Worker will refer Veterans and their families/caregivers to the appropriate interdisciplinary team member for identified health education needs. Uses sound judgment and critical thinking and has strong teamwork and communication skills. In unusual and/or complex administrative and clinical problems, the incumbent initiates consultation with the supervisor. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ? 7403(f). Grade Determinations: Senior Social Worker, GS-12 Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.(b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.(c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.(d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.(e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: Experience working with the geriatric population. References: VA HANDBOOK 5005/120 PART II APPENDIX G39. The full performance level of this vacancy is GS-12. Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies such as papers, books, or small parts. The incumbent must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the applicant or others. The incumbent must be able to drive an automobile. This position also requires close visual acuity (near vision correctable at 13" to 16"} to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers. Use of fingers, both hands required, ability for rapid mental and muscular coordination simultaneously, hearing (aid permitted), and clear distinctive speech are all required.
