This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

Current permanent VA employees and Federal employees from other federal agency should apply under CBST-12065347-23-TC. Major duties include, but are not limited to: Incumbent performs specialized treatment of complex physical or mental illness, including adjustment to chronic or life threatening diagnoses. Incumbent incorporates complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Incumbent communicates with team members to ensure coordinated treatment program and ensure timely and cost-effective outcomes. Acts as a consultant and resource for community agencies and other VA Medical Centers. Provides educational information to veterans and their families regarding end stage renal disease and treatment related issues. Educates referral sources, community agencies, Veteran Service Organizations and other interested parties regarding the scope and management of this patient population. Participates in goal sharing groups that address performance improvement monitors. Administers and utilizes information, such as the National Kidney Foundation-Kidney Disease Outcomes Quality Initiative (NKF KDOQI), and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. The Renal Coordinator is assigned administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns related to ESRD case management and referral services. Incumbent must have knowledge of the guidelines used by social security for eligibility for Medicare part B for End Stage Renal Disease patients. Incumbent must also have knowledge about the eligibility and application process for the Texas Kidney HealthcareProgram, American Kidney Fund, and the Department of Health/ERSD Network 14 Rules and Regulations. Incumbent must also have knowledge about the VAMC criteria for 100% SC End Stage Renal Disease (ESRD) veterans and fee basis programs. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education- Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure- Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency Requirement - Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j. GS 12 Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Experience assessing the psychosocial functioning and needs of Veterans and their family members from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds. Experience utilizing supportive, problem solving, or crisis intervention techniques. Experience developing and implementing methods for measuring effectiveness of social work practice and services utilizing outcome evaluations to improve treatment services and design system changes. References: VA Regulations, specifically VA Handbook 5005/120, Part II, Appendix G39 Social Worker Qualification Standard. Physical Requirements: The work is primarily sedentary. The employee may sit comfortably in an office or clinic to perform the work. However, there may be some walking, standing, stooping, bending, and carrying light items such as papers, books, etc.
