This position is located in Cedar Rapids Iowa at the Community Resource and Referral Center. The incumbent will service as a HUD-VASH Geriatric Coordinator for the VA Homeless Program. This is a collaborative position that works closely with both the HUD-VASH and Geriatrics and Extended Care National Programs.

Duties

Program Development Participate in the development, evaluation, and communication of standards, policies, procedures, and plans of care for partnerships serving aging and disabled HUD-VASH Veterans, including partnerships between GEC and HUD-VASH programs. Educate VISN and Medical Center level staff in Homeless Programs regarding various models for GEC and HUD-VASH partnerships Evaluate GEC and HUD-VASH partnerships using relevant VA statutes and regulations and take action to improve identified deficiencies. Screening/Assessment Assist with needs assessments across the site to support in program development identification. Referral to Service Providers Serve as liaison between Homeless programs, GEC programs, and other community programs to develop programming and housing designed to serve the aging and/or disabled homeless Veteran population. Serve as the subject matter expert on VA and community based homeless and geriatric services and systems of care. Responsible for educating the VISN and individual medical centers on the available resources and options to develop programming and housing across homeless programs and GEC. Resource Development Responsible for developing and sharing resources related to VA and community social service programs to support GEC and HUD-VASH program development. Responsible for partnering with Public Housing Authorities, the Office of Asset and Enterprise Management, and community partners to develop project-based HUD-VASH housing and other appropriate housing opportunities for aging and disabled HUD-VASH Veterans. Advocacy Understand the intimidation of bureaucracy and will act as an advocate when it serves the best interest of Veterans. Educate and encourage GEC and homeless programs to advocate as appropriate within the medical center to develop/improve programming. Administrative Responsibilities Establish and maintain positive working relationships with internal and external customers. Attend National, VISN, and medical center meetings as appropriate. Collaborate in the performance improvement processes and comply with performance measures as required by the VA for these specific programs. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Loss of Licensure or Certification. Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS 12 Grade Determinations: Experience and Education. One year of experience equivalent to the GS-11 grade level. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Experience working with Aging Veterans and Homeless Veterans. References: VA HANDBOOK 5005/120 PART II APPENDIX G39 The full performance level of this vacancy is GS 12. Physical Requirements: Work is divided between office, clinical and community settings. May require prolonged standing, moderate walking, and manual dexterity. Requires carrying light items, such as books, files, or laptop computer. Extensive community work is involved with this position. Driving Requirements. Drivers may be expected to transport veterans and must hold a current valid driver's license. All drivers must meet the minimum VA and VHA medical requirements for the operation of vehicles. Medical evaluations of motor vehicle operators and incidental operators must be conducted.
