The Outreach Coordinator for VA's Health Care for Homeless Veterans (HCHV) Program is responsible for coordination of direct and indirect outreach services and supervision of HCHV social workers and Liaisons.
Duties
The HCHV Outreach Coordinator is responsible for supervision of the Coordinated Entry Specialist, HCHV Social Workers and the Liaisons and responsible for the data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds, and performance measurements for the HCHV program. Additionally, the HCHV Coordinator develops working relationships and agreements with other organizations, as needed.
The HCHV Coordinator organizes and provides oversight for The National Call Center for Homeless Veterans, ensuring contacts and interventions provided are clinically appropriate and follow national and local policies and outreach services, including Stand Down and other outreach events for the homeless population.
The incumbent is responsible to perform difficult and complex advisory/assistance and/or coordination functions that require interactions between many entities and has to independently exercise clinical judgment to ensure the health and safety of veterans living in unsafe conditions.
The incumbent coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, linking, and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies. The incumbent engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless veterans and advocates for expanded services and coordination by acting as a subject matter expert regarding homeless veteran care needs. The incumbent will provide education regarding current information on services and benefits available through the VA's and community at large to ensure care is coordinated throughout the VA catchment area.
The incumbent serves as the Community Homeless Assessment, Local Education and Networking Groups (CHALENG) point of contact.
The incumbent actively participates in the HCHV National and VISN Conference Calls and attends appropriate VA Homeless Program trainings.
The incumbent actively participates in program planning and development, advising the HCHV Coordinator and Homeless Programs Social Work Supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences and provides consultation to other hospital personnel and
/or community agencies as needed.
The incumbent actively participates in the in all HCHV/ GPD(Grant Per Diem) and CES(Coordinated Entry Specialist) National calls.

Qualifications

Basic Requirements:
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Experience/Education: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills in administration, demonstrating progressively more professional competency skills and judgment. Individual may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: The social worker performing research must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.
