The Facility Program Manager for the Pharmacogenomics is a licensed pharmacist with knowledge/proficiency in pharmacy practice and program management. The incumbent functions at an advanced level of clinical practice, working independently under their scope of practice in the area of pharmacogenomics, providing direct support for Pharmacogenomics activities. NOTE: Recruitment/Relocation incentives with 2 year service agreement may be approved for highly qualified candidates.

Duties

The Facility Program Manager for Pharmacogenomics is a licensed pharmacist with knowledge and proficiency in pharmacy practice and pharmacy program management. The incumbent is knowledgeable in the area of Pharmacogenomics and will provide direct support for Pharmacogenomics activities within the health care system. The Program Manager performs higher level duties consisting of significant scope, administrative-independence, complexity and range of variety at least 25% of the time. Duties may include but are not limited to: Program Management/Educational/Administrative: Implements approved policies designed to support the principles of Pharmacogenomics. Provides education to health care professionals using academic detailing frameworks about Pharmacogenomic principles and appropriate indications and contra-indications for cost-effective Pharmacogenomic testing. Provide resources and education that empowers patients to make an informed healthcare decision about undergoing Pharmacogenomic testing and understanding their test result. Lead diffusion of the Pharmacogenomic pharmacist train the trainer program to offer training to pharmacy leaders, clinical pharmacists, clinical pharmacy practitioners, and pharmacy residents. Facilitate that Pharmacogenomic test results are handled in an ethical manner and that patients are provided access to their genetic data when applicable. Serve as subject matter expert in Pharmacogenomics for assigned committees, workgroups and interdisciplinary teams. Clinical - Direct Patient Care Activities (1) Functions as an advanced practice provider for CMM services to achieve defined outcomes through direct interactions with patients and providers in assigned care area(s). Prescribes, monitors, and adjusts medications related to Pharmacogenomics management. Obtains pertinent medical history and obtains accurate medication related history and Pharmacogenomic family history from patient and via chart review. Orders Pharmacogenomic testing when indicated to aid in the process of medication and dosage selection or deprescribing. Evaluates third party pharmacogenomic testing results to determine clinical validity and utility for the veteran served. Utilizes race-agnostic Pharmacogenomic dosing guidelines and algorithms in accordance with PharmGKB statement on eliminating race-based medicine. Identifies and reconciles drug-gene, drug-gene-disease, and drug-drug-gene (i.e., phenoconversion) interactions. (2) Performs continuous evaluation of prescribed medications to assure optimal drug therapy and positive patient centric outcomes. Clinical-Indirect Patient Care Activities Serves as a drug information resource and consultant for intensive medication therapy management services by providing up-to-date drug information to prescribers, other health care professionals, patients and caregivers. Participates in team meetings, reviews charts, orders and evaluates pertinent laboratory and other tests, assesses drug-drug and drug-nutrient interactions, monitors for adverse drug effects, and screens for allergies. Facilitate the creation of a pharmacist-led specialty Pharmacogenomics service using either consult, e consult, or care coordination agreement (CCA) functionality. Review relevant population management dashboards to identify patients who may benefit from engagement in care for Pharmacogenomic testing, management, and/or risk mitigation. Assesses drug safety and efficacy, including evaluation of physical symptoms and performance of physical assessments. Reports adverse drug events (ADE), near misses, and medication errors in alignment with VAADERS Adverse Drug Reaction (ADR) reporting program. Dispensing and Compounding Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, prior to dispensing. Ensures all medication orders are entered into the electronic patient medical record. Processes, checks and fills all orders in a timely fashion; review of the patient's allergy history, drug- drug, drug-nutrient, drug-laboratory & drug-disease state interactions & assessment for clinical relevance should be performed & dealt with appropriately. Reviews medication profiles and monitors for compliance or potential abuse; problems encountered shall be resolved. Supervises the activities, provides oversight of support personnel & technical staff in all aspects of medication distribution. Work Schedule: 8:00 am -4:30 pm - Weekend and nights rotated on a fair and equitable basis based upon service needs. Telework: Available - Per Agency policy and Service Approval on an Ad hoc basis. Education (Transcripts Submitted): Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT) Licensure (Copy Submitted): Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence, or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. ? 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration, or certification. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Licensed Pharmacists may qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations for Clinical Pharmacy Specialist - GS-0660-13 (Above full performance): Experience: In addition to the Basic Requirements, Pharmacists assigned to the GS-13 grade must have 1 year of experience equivalent to the next lower grade level (GS-12): Assignments: For all assignments above the full performance level, the higher the level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Facility Program Manager. Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organized, manage, and control complex pharmacy programs. Develops, organizes, coordinates, and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is a reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Knowledge, Skills, and Abilities (KSA's): Pharmacists assigned to this position must demonstrate the following KSAs: (a) Ability to communicate orally and in writing to persuade and influence clinical and management decisions. (b) Expert understanding of regulatory and quality standards for their program area. (c) Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. (d) Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. (e) Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. References: VA Handbook 5005/55, Part II, Appendix G15, Licensed Pharmacist Qualification Standard Preferred Experience: Has completed a PGY1 Pharmacy Practice Residency or has equivalent experience. Ideal candidate will have obtained a Pharmacogenomic Certificate from a reputable educational source (e.g., ASHP, ACCP, etc.). Demonstrate the ability to communicate orally and in writing to persuade and influence clinical and management decisions. Understand regulatory and quality standards for their program area. Demonstrate strong/effective program management skills. Possesses expert knowledge within clinical pharmacy practice and pharmacogenomics. Advanced skill in monitoring and assessing the outcome of drug therapies, including interpretation of laboratory and other diagnostic parameters The full performance level of this vacancy is 13. Physical Requirements: The work requires regular and recurring physical exertion, involving stating or sitting for prolonged periods of time, typing, walking. Additionally, occasional lifting/carrying of pharmaceutical supplies may be required, and bending, stooping or stretching.
