The Clinical Pharmacy Specialist serves as clinical pharmacy specialist expert and consultant to the Chief of Pharmacy Service for medication related issues in the Preventive Medicine Clinic. Principal responsibilities include pharmacotherapy management, provision of drug information services, and the development and supervision of pharmacy programs for the Preventive Medicine Clinic.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Major Duties: The Clinical Pharmacy Practitioner (CPP), Preventive Medicine, utilizes expertise in the area of Preventive Medicine to provide medication management to optimize patient care. As an advanced practitioner of Preventive Medicine he/she is authorized within the guidelines of a scope of practice to function as a provider of services to patients of the VA TVHS requiring minimal supervision in both the practice area of Preventive Medicine. Principal responsibilities include evidence-based pharmacotherapy management (including identification of patient specific medication issues such as drug interactions, adverse effects, lack of effectiveness, appropriateness, and compliance problems), and a provision of drug information service to medicine and specialty services both inpatient/outpatient and to other areas within Clinical Pharmacy Service as requested. Is responsible for planning, developing, and maintaining effective clinical pharmacy programs by implementing rational cost-effective drug therapy to improve patient outcomes within medicine and specialty services. Provides medication information to medicine and specialty team members as required. Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurances activities as required to ensure appropriate and safe drug utilization. Assists in initiation of medication use improvement strategies including development of quick order sets, clinical reminders, patient/provider education handouts, and development and implementation of pharmacotherapy protocols. Reports directly to and requires minimal supervision by the Primary Care/Outpatient Clinical Pharmacy Programs Supervisor. Assures compliance with VA and other accrediting bodies in terms of meeting all medication use standards. Actively participates in local, state, and national organizations related to the advancement of the pharmaceutical profession. Performs other duties as assigned. Clinical Practice: Functions as an advanced practice provider for CMM services to achieve defined outcomes through direct interactions with patients and providers in assigned care area(s). Utilizes computer expertise in interpretation of national dashboard analytics or development of local data extraction tools to help guide evidenced based prescribing for chosen disease sates. Makes recommendations to the responsible physician for optimal management of drug induced problems either verbally or in writing when requested or initiates changes as specified under a scope of practice. Participates in the appropriate, effective, and safe use of pharmaceuticals by contributing to criteria development, data collection and drug use evaluation and other quality improvement activities as assigned. Education: Serves as a pharmacotherapy expert expert and mentor to medical students, residents, and physician extenders, teaching them pharmacotherapy and healthcare economics. Participates in P&T Committee functions and assignments as required (e.g., ADR monitoring, medication error reporting, Medication Utilization Evaluation activities, drug monographs/reviews, annual order set and protocol reviews, etc.). Research: Participates in design and/or completion of research studies and/or quality improvement projects involving drug therapy, clinical pharmacy functions, or improvements in patient care and presents it either at national meetings or publishes it in professionally representative publications. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Grandfathering Provisions: All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. Grade Determinations GS-13: Experience: In addition to the basic requirements, applicants must have one year of experience equivalent to the next lower grade level (GS-12). For all assignments above the full performance level, the higher-level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: 1) Doctor of Pharmacy degree plus (1), (2), or (3), below: 1) Completion of ASHP accredited PGY-1 Pharmacy Practice residency and Three (3) years of practice with substantial component of patient care activities acting as a mid-level provider in the specialty area of primary care and/or preventive medicine. 2) Completion of BOTH an ASHP accredited PGY-1 Pharmacy practice and a PGY-2 Ambulatory Care Residency with focus in chronic disease state management and three additional years of practice with a substantial component of patient care activities serving as a mid-level provider in specialty area of primary care and/or preventive medicine. 3) Completion of BOTH an ASHP accredited PGY-1 Pharmacy Practice Residency and a AHSP PGY-2 Specialty Ambulatory Care Residency with a substantial component of patient care activities during the residency, serving as a mid-level provider in specialty area of primary care and/or preventive medicine. References: VA Handbook 5005/55, Appendix G15. VA Licensed Pharmacist Qualification Standard GS-0660, dated March 12, 2021. This can be found in the local Human Resources Office. The full performance level of this vacancy is GS-13. Physical Requirements: Pre-employment physical is required. The work requires some regular and recurring physical exertion, involving standing or sitting for prolonged periods of time, typing, walking distances within the medical center, lifting/carrying (up to 40 pounds) of pharmaceutical supplies, and bending/stooping/stretching. Mental and emotional stability, working closely with others, normal ranges of hearing and vision (corrected, or with use of aids) ability to effectively communicate, and the ability to distinguish between basic colors and shades of color are required.
