This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

Incumbent serves as the Clinical Pharmacy Specialist, an independent practitioner in the primary care setting requiring the highest level of pharmacist clinical privileges with minimal supervision. Principal responsibilities include pharmacotherapy management, provision of drug information services, and the development and supervision of pharmacy programs in the primary care firm. Incumbent has the credentials required for approval of a Scope of Practice with prescribing authority by the VA Tennessee Valley Healthcare System Credentialing Committee. Duties include, but not limited to: Clinical Practice Practices as the Clinical Pharmacy Specialist for the assigned primary care area. The Clinical Pharmacy Specialist supervises all pharmacotherapy provided by team providers to ensure that National Treatment Guidelines are being followed in addition to adherence to National Formulary and local policy. The Clinical Pharmacy Specialist will independently manage unstable pharmacotherapy patients in his/her pharmacotherapy clinics, improving the clinic system efficiency and quality. Utilizes the highest level of clinical privileges for Clinical Pharmacy Specialists to manage assigned patients. Interfaces with providers, including physicians, PAs, ARNPs in the design of the Veteran's treatment regimen to achieve optimization of rational and cost-effective drug therapy. Serves as an authoritative information source on drugs and their utilization in therapy, with special emphasis on medications used in treatment of the assigned patients. Education Educates patients on the proper use and care of their medications through the day-to-day personal interactions with the patients. Provides formal education and training programs in the area of primary care disease states pharmacology, therapeutics, pharmacokinetics and related subjects to trainees, physicians, nursing staff and other health care professionals as appropriate. Contributes to Pharmacy Newsletters and other written educational written educational materials as assigned. Participates in P&T Committee functions and assignments as required, e.g., ADR monitoring, medication error reporting, Medication Utilization Evaluation activities, drug monographs/reviews, etc. Provides drug information and drug literature evaluations as requested by the professional staff. Research Serves as preceptor of resident's research projects as assigned. Program Management Responsible for coordination and supervision of all pharmacotherapy management activities in Primary Care as assigned. This includes management of a clinical practice within area of expertise including scheduling, reviewing consults, providing patient care, and assuring coverage of clinical services. Quality Management Participates in the appropriate, effective and safe use of pharmaceuticals by contributing to criteria development, data collection and drug use evaluation and other quality improvement activities as assigned. Actively filters through eQM data for appropriate patients for enrollment into the Clinical Pharmacy Specialist Pharmacotherapy clinic. Qualifications

Basic Requirements: Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) OR Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Pharmacist - Clinical Specialist, GS-13 (Above Full Performance Level) Experience. In addition to the basic requirements, candidates must have 1 year of specialized experience equivalent to the next lower grade level (GS-12) to qualify for the GS-13 position. AND; Knowledge, Skills, and Abilities. To be creditable, the experience must demonstrate the following knowledge, skills and abilities (KSAs): Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Doctor of Pharmacy degree, plus (1), (2), or (3) below: Completion of an American Society of Health-System Pharmacists (ASHP) accredited PGY-1 Pharmacy Practice Residency and three (3) years of practice with substantial component of patient care activities acting as a mid-level provider in the specialty area of primary care. Completion of BOTH an ASHP accredited PGY-1 Pharmacy Practice Residency and a PGY-2 Ambulatory Care Residency with focus in chronic disease state management and three (3) additional years of practice with a substantial component of patient care activities serving as a mid-level provider in specialty area of primary care. Completion of BOTH an ASHP accredited PGY-1 Pharmacy Practice Residency and an ASHP PGY-2 Specialty Ambulatory Care Residency with a substantial component of patient care activities during the residency, serving as a mid-level provider in a specialty area of primary care.
