The position functions as the Senior Case Manager for Battle Creek VAMC's Housing and Urban Development-VA Supportive Housing (HUD-VASH) Program. The HUD-VASH Program falls within the parameters of the Healthcare for Homeless Veterans (HCHV) Program. The incumbent will report to the HCHV Supervisor or designee.
Duties
The major duties include:
Provide case management for an assigned caseload of Veterans in the HUD-VASH Program.
Review agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements.
Performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes.
Verifies the Veteran's status, eligibility for VA medical care, and eligibility for Program participation.
Collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures.
Providing HUD-VASH case management.
Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies for identified services.
Identifying the need for and making recommendations on Program procedures or policy changes based upon work experiences.
Document all contacts with Veterans/families in Computerized Patient Record System (CPRS);
Completes statistical and other reports in accordance with VA Medical Center and respective professional discipline service policies.
Conduct psychosocial assessments to determine the needs of Veterans and family members.
Provide a full range of social work services within commonly accepted standards of social work practice which include case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.
Maintain data and statistical compilations to comply with JC, CARF, VA, and Medical Center policy and procedures.

Qualifications
Grade Determinations: In addition to the basic requirements for employment, you must meet the following grade requirements.
Senior Social Worker, GS-12
Experience/Education: You must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Licensure: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and you must be able to provide supervision for licensure.

Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice.

Physical Requirements: The work is mostly sedentary. It typically requires sitting with sustained periods of concentration. There will be some walking, standing, bending, and carrying of relatively light objects. The position may also require stooping, reaching, lifting, pulling, and other moderate exertion or strain. Incumbent may occasionally have to walk some distance to a meeting. The incumbent will also be required to perform (50%) of their job duties in the community and will be required to utilize a GSA Vehicle while performing such duties.
