This position is responsible for the delivery of Whole Health Coaching and skill development, including provision of care through coaching and promotion and education to Veterans and staff. The position is located within the Nursing Department at the VA San Diego Healthcare System. Duty location may be assigned to the main facility in La Jolla or a Community Based Outpatient Clinic in Kearny Mesa.
Duties
Duties include, but are not limited to:
Contacts eligible participants for enrollment and explain the health coaching process.
Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones.
Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings.
Coach/assist clients to use their insight, personal strengths and resources, goal setting, action steps and/or accountability toward healthy lifestyle change.
Participate in leadership roles on local, state and/or national professional organizations specific to health coaching.
Schedule ongoing sessions to assess progress and assist in navigating obstacles or challenges.
Utilize motivational interviewing and health coaching techniques and processes, learned through pre-requisite health coach training, to assist members in developing a realistic plan to accomplish their goals.
Utilize VISTA, CPRS, Microsoft office software, Intranet and Internet and other applicable software to track and/or support patient progress.
Guides Veterans to learn about available support tools in the VA and the community that assist in attainment of goals.
Conduct Facilitated Groups; Taking Charge of My Life and Health & Intro to Whole Health
Facilitated Movement classes for Veterans.
Developed mindfulness classes for Veterans.
Fosters an environment of individual responsibility to maximize each Veteran's ability to adhere to a lifestyles management plan.
Coordinates with others on the Veteran's health care team to ensure optimal patient care.
Performs other related duties as assigned.
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.
Work Schedule: Full-Time, Monday - Friday 0730 a.m. -0400 p.m.

Physical Requirements: Regular and recurring work is mostly sedentary involving sitting at a desk, although the position requires the ability to move from one geographic location of the facility to another. Some physical effort such as standing, walking, bending prolonged sitting may be required. The work places no special physical demands of the employee.

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 07/24/2023.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Proficiency: English Language Proficiency Requirement: Per VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English.
GS-07 Specialized Experience: You must have at least one (1) full year of specialized experience equivalent to at least the next lower grade level (GS-5) in the Federal Service, that is directly related to the work of the position and has equipped you with the particular knowledge, skills and abilities to successfully perform the duties as a Social Science Program Coordinator. Qualifying specialized experience includes: provides health coaching for Veterans; develops treatment plans; provide educational opportunities for staff; data management analyses; knowledge of abstracting reports for use by management for budget preparation, workload projections, and quality assurance and utilization reviews.

Education: Successfully completed one (1) year of graduate-level education or superior academic achievement in the related field of the position to be filled. The education portion must include courses that demonstrate the knowledge, skills, and abilities necessary to do the work of this position. Education must have been obtained in an accredited college, or university. NOTE: Transcripts must be submitted with your application materials. Education cannot be credited without documentation.
OR
Combination: Combinations of successfully completed undergraduate level education and specialized experience as described above to meet total experience requirements. The education portion must include graduate courses that demonstrate the knowledge, skills, and abilities necessary to do to the work of this position. GS-09 Specialized Experience: You must have at least one (1) full year of specialized experience equivalent to at least the next lower grade level (GS-7) in the Federal Service, that is directly related to the work of the position and has equipped you with the particular knowledge, skills and abilities to successfully perform the duties as a Social Science Program Coordinator. Qualifying specialized experience includes: reviewing supporting material to determine compliance with regulations and policy requirements; assists in planning and implementing program(s) within a medical center setting; using creativity to engage employees and community partners in a newly developed program; developing training materials for staff educational needs; and experience using a wide range of qualitative and quantitative methods for the assessment and improvement of program effectiveness.

Education: Successfully completed two (2) years of progressively higher-level graduate education leading to a master's degree or master's or equivalent graduate degree in a related field of the position to be filled. Education at the graduate level must have been obtained in an accredited college or university and must demonstrate the knowledge, skills, and abilities necessary to do the work of this position. OR
Combination: Combinations of successfully completed graduate level education (beyond the first year) and specialized experience as described above, to meet the total experience requirements.
