The Clinical Pharmacy Technician (CPhT), Primary Care Service is responsible for using dashboards to track clinic productivity; assisting with continuity of care including no-shows, patients lost-to-follow up and obtaining outside records; providing telephone support, laboratory value monitoring, electronic health record reviews, supplemental patient and provider education communication, medication reconciliation and prescription order entry (with pharmacist verification).
Duties
The Clinical Pharmacy Technician (CPhT) will be under the supervision of the Associate Chief of Pharmacy for Primary Care and daily oversight by primary care (PACT) pharmacists, the CPhT will be responsible for all aspects of Primary Care Pharmacy Clinic support as defined by VA Northern California Health Care System.
Duties may include:
Electronic Health Records (EHR):
Navigate sections including progress notes, pharmacy, laboratory, immunization record, and allergy information to identify pertinent information.
Identify basic problems, such as duplicate therapy with drugs in the same class and alert the Clinical Pharmacist Practitioner (CPP) to the possible need for clinical intervention.
Place orders for CPP verification.
Document in the EHR and send laboratory reminder letters to patients.
Dashboards:
Generate reports from Primary Care, Pain, and other population health dashboards as assigned and communicate findings to CPP.
Monitor workload dashboards to support CPP productivity.
Clinic Support:
Triage messages with escalation to CPP as needed.
Request/review outside records and update VA's EHR.
Process medication refills upon request and assists with troubleshooting and tracking prescriptions as needed.
Assist with medication inventory and reconciliation.
Assist with resulted labs including communicating normal results to patients as requested.
Follows up on requests in a timely manner ensuring the needs of the customer are met while also promoting harmonious relationships within the team.
Care coordination:
Track newly assigned PACT patients to ensure pre-clinic appointment with CPP is arranged.
Follows up to ensure scheduling for lab, telephone, VVC, and clinic visits when requested.
Track no-shows, cancellations, lost to follow-up to re-establish care, and two-day post discharge report and communicate findings to CPP.
Scan discharge summary into VA's EHR when discharged from non-VA hospitals as requested.
Professional activities:
Participate in self-development programs, assigned meetings, and assigned student teaching activities.
Assist in total quality improvement activities by participating in medical center and service level processes and performance improvement teams and continually seeks to improve service.
Work Schedule: Monday through Friday 8:00 AM to 4:30 PM
Telework: Telework, if approved, would be on an AD-HOC basis.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education or Experience. A high school diploma or equivalent; or at least 3 months experience.
Certification. Certification by the National Pharmacy Technician Certification Board (PTCB) is required at grades GS-6 and above.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-7
Experience. One year experience at the next lower level and must fully meet the KSAs at the next lower level. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*):
*1. Ability to evaluate, analyze workflow, and coordinate work activities.
2. Ability to independently gather, evaluate, and analyze data to generate reports and/or for other projects/statistical purposes.
3. Knowledge of pharmacy policies and procedures sufficient to train and orient new employees and other staff
4. Comprehensive knowledge of the operation and care of specialized pharmaceutical equipment.
5. Comprehensive knowledge of the procurement and formulary process.
6. Comprehensive knowledge of investigational drug policies and procedures.
7. Comprehensive knowledge of controlled substance policies and procedures, and the ability to identify and investigate discrepancies in these policies and procedures.
8. Comprehensive knowledge of, and ability to, analyze data in VA information systems.
Preferred Experience: recent experience in a current or previous job that includes working successfully in an interdisciplinary team setting, independent completion of projects, excellent customer service and written/oral communication skills, and experience working with Microsoft Office applications. Preference will be given to candidates with experience working with the primary care practice setting.

The full performance level of this vacancy is GS-07.
Physical Requirements: The position requires visual and auditory acuity at a level to provide safe and effective care. Use and function of all normal motor and sensory anatomy for provision of patient care, reading, writing, listening, observing/seeing, heavy lifting, bending, pulling, pushing, stooping, squatting, walking, standing, use of fine motor muscles. In an eight-hour day, must be able to sit for 3 hours, stand for 3 hours, and walk for 1 hour. The position also requires ability to work on computers for extended periods of time. Requires use of hands for actions such as typing, simple grasping, fine manipulation, and repetitive actions. Requires the ability to frequently respond to multiple demands and priorities; emotional intelligence to respond to stressful and/or emergent situations and crises appropriately and calmly; self-care skills to maintain physical, psychological, and emotional health. Heavy lifting of 35 pounds and over, heavy carrying of 35 pounds and over, straight pulling, pushing, reaching above shoulder, use of fingers, both hands required, walking, standing, kneeling, repeated bending, ability for rapid mental and muscular coordination simultaneously. Has potential for exposure to infectious diseases, hazardous waste, and chemicals.
