Duties
Serve as a Clinical Pharmacist Practitioner facilitating pharmaceutical care services and direct patient care programs to ensure drug utilization activities are aligned with patient care needs, best practices, and regulated standards.
Maintain collaborative and comprehensive medication therapy management clinics and services in designated areas of practice, under a scope of practice.
Deliver direct care to patients though a variety of modalities and ensure documentation for encounters and clinical decisions in the electronic health records.
Communicate effectively and professionally with patients, and other members of the healthcare team with the goal to educate, promote patient safety, and practice advancement.
Review electronic medical record to monitor for drug therapy, including evaluation for adverse drug reactions, drug and drug food interactions, disease due to iatrogenic cause, medication therapy duplication, or unnecessary polypharmacy.
Identify high-risk patients and implements interventions to improve quality and safety.
Perform drug usage evaluations, quality assurance/ performance improvement activities as required.
Requirements

- This position requires the successfully completion of a pre-employment Physical Examination and an annual examination thereafter.
- This position is a Testing Designated Position (TDP) subject to a pre-employment drug screening, and random testing thereafter, to include testing based on reasonable suspicion and testing due to direct involvement with an on-duty accident.
- Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
- Selectees will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption.
- Must be able to obtain and maintain current Basic Life Support (BLS) certification through the American Red Cross. Advanced certification (e.g., ALS) does not supersede BLS.
- This position may require the incumbent to work on call during irregular work hours which may include evenings, weekends, holidays, and/or varying work schedules.
- Appointment to this position is subject to a one-year probationary period unless the appointee has previously met the requirements as described in 5 CFR Part 315.
- This position has a Temporary Duty (TDY) or business travel requirement of 5% of the time.
- This position has been designated "Mission Essential" in case of severe weather conditions or other such emergency type situations, incumbent is required to report to work or remain at work as scheduled to support mission operations.
- Employees assigned to a medical treatment facility are required to be screened for applicable immunizations or pre-existing immunity based on occupational risk, regardless of job series unless exempted for medical or administrative personnel.

Basic Requirement for Pharmacist:
Degree: A doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Exemption: You are exempt from the basic requirement above if you are a current federal employee occupying a position in the 0660 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status.
Licensure: A current, active, valid, and unrestricted license to practice pharmacy in a State, a territory of the United States, or the District of Columbia.
Medical Requirement: Must be able to distinguish basic colors.
Experience Required: One year of professional pharmacy experience which includes providing clinical pharmaceutical care to patients; advising on medication therapy management; and training or educating health care professionals on the use of drugs and other pharmaceuticals. This definition of experience is typical of work performed at the next lower grade/level position in the federal service GS-12.
Education
