The Sheridan VA Health Care System, with its Community Based Outpatient Clinics, is a complexity level 3 facility serving Veterans across three quarters of Wyoming and the Rocky Mountain Region. The Sheridan VA Health Care System's main facility, and it is a 185-bed medical center, with 54 buildings on a 296-acre campus. Our primary catchment area extends to veterans from Wyoming, Montana, Colorado, Utah, and portions of Idaho and serves as the psychiatry tertiary referral site for VISN 19.
Duties
Establishes and maintains an ongoing education program for Veterans, caregivers, community agencies, students, and staff to facilitate understanding of clinical practice guidelines, mental health issues, readjustment issues and psychosocial issues facing caregivers.
Develops and facilitates CSP-program focused direction, orientation, coaching, in-service training, staff development, and continuing education programs for VA facility staff and the community.
Develops and implements a plan for outreach to Veterans and Veteran caregivers who may benefit from the CSP program services.
Maintains a current network of referral resources to include peer support mentoring groups, caregiver skill building and self-care educational opportunities, mental health support, legal and financial services.
Directly supervises the VA HCS Caregiver Support Coordinator(s) (CSC) and, when applicable, Program Support Assistant (PSA) and other interprofessional staff assigned to Caregiver Support Program as outlined in VHA Directive 11152(1), Caregiver Support Program.
Coordinates program implementation with multiple clinical services across all campus locations within the VA facility, including but not limited to: nursing; physical, occupational, and speech therapists; mental health and primary care providers.
Provides clinical interventions to the Veteran and caregiver populations served by the programs.
Subject matter expert (SME) to VA Facility leadership and staff for all CSP related issues and serves as a consultant to other VA professionals and administrators on program specific activities.
Serves as a liaison to Veterans Integrated Service Network Program (VISN) CSP Program and the VA Central Office (VACO) Program.
Serves as the VA HCS POC for all inquiries and outreach requests regarding eligible PCAFC and PGCSS caregivers.
Identifies unmet needs of the caregiver population, their families/significant others and caregivers.
Works collaboratively with facility, VISN, and VA Central Office staff to develop and implement solutions-participating in national, regional, and VISN operational calls; contributing to national leadership council committees; and implementing cultural transformation initiatives.
Performs the administrative and personnel management function relative to staff supervised including personnel actions, clinical supervision, consultation, negotiation with other departments, and quality improvement.
Responsible for monitoring performance measures at the local, VISN, and Central Office levels; providing responses to congressional inquiries; and briefings to providers, VA leadership, community organizations and other internal and external stakeholders.
Oversees the daily operations of the CSP program.
Develops policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use.
Manages CSP clinical and business operations, accountability for program effectiveness, and modification of service patterns to comply with the delivery of standardized processes found within Directive 1152(1).
Supports customer satisfaction, wellbeing and service delivery.
Addresses Congressional inquiries, VISN, and VA Central Office action items as requested.
Develops a data driven, continuous quality improvement program with established goals and outcomes to evaluate and document the program's effectiveness.
Develops a system to identify family caregivers served by the VA facility including caregivers in the catchment area not enrolled in CSP.
Recognizes gaps in workforce competencies and ensures supervised staff who require knowledge and skill development receive the training to necessary to execute their duties.
Certifies staff completion of all required annual and ad hoc trainings through documentation and reporting.
Plans, organizes and coordinates administrative, operational, clinical and personnel activities for the CSP program.
Responsible for all aspects of program staffing, work assignments, budget and treatment services.
Responsible for managing and reporting on program specific fund control points, labor mapping, workload, and clinic utilization within the assigned program area.
Manages and controls the use of data related to these programs regarding day-to-day functioning and program evaluation.
Manages all aspects of the PFCAFC and the PGCSS to ensure eligible caregivers have awareness and access to the national Caregiver Support Line (CSL), VHA's Peer Support Mentoring Program, self-care and resiliency building trainings and personal care service skill development trainings.
Performs other related duties as assigned.

Qualifications
Basic Requirements:
Citizenship - You must be a citizen of the United States
English Language Proficiency - You must be proficient in spoken and written English as required by 38 U.S.C. 7403(f)
Driver's License - Must Posses a Valid Unrestricted State Issued Drivers License
Education and/or Experience Combination -
You must possess a master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work.
License or Certification - You must be licensed or certified by a state to independently practice social work at the master's degree level.
Social Worker Program Coordinator GS-0185-13
Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure Requirement: You must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs.
Skill in coordinating and implementing one or more specialty program(s), across the facility.
Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.
Knowledge of organizational structure and impact on program operations.
Preferred Experience: Supervisory Experience, Program Management, Gathering and report program metrics, Working with Multidisciplinary Teams, Budgeting, Meeting implementation timelines, Program Development.

Physical Requirements: This position provides direct services to patients and their families through casework. Incumbents must be physically and mentally able to efficiently perform the essential functions of the position without hazard to themselves or others. Normal work of the position requires visual acuity (with the ability to read typewritten characters without strain, corrective lenses permitted), keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially moderate levels of lifting and carrying (15-44 pounds). The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia,
or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. Must have a valid driver's license in a state. May be expected to operate Government Vehicle.
