This Job Opportunity Announcement will be used to fill a Clinical Pharmacy Specialist, GS-0660-13 position in Pharmacy Service with the Eastern Oklahoma VA Health Care System, Muskogee, OK.
Duties
The Clinical Pharmacist Specialist (CPS) functions at the highest level of clinical practice and works independently under their individual scope of practice to directly care for patients. A CPS functions as an Advanced Practice Practitioner who may be assigned to the following areas: Mental Health, Ambulatory Care, Acute Care Medicine, Anticoagulation, Antibiotic Stewardship, Surgery, Emergency Room, and Specialty areas. The CPS may initiate, modify, or discontinue medication therapy and as a consultant for intensive medication therapy. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. The CPS will have knowledge of best pharmacy practices, ASHP Policy Positions and Guidelines, The Joint Commission requirements, Veterans Affairs (VA) performance measures, Pharmacy Benefits Management (PBM) initiatives and the ability to integrate these standards into practice
Major duties and responsibilities of the position include but are not limited to:
Functions in a Clinical Pharmacy role providing appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate. The CPS will provide these services while functioning under a Global Scope of Practice. The Scope of Practice for a CPS must allow for initiation, modification, and discontinuation of medications based on the principles previously listed.
Provides patient-specific therapeutic drug monitoring and communicates relevant findings and /or recommendations to other healthcare providers in charge of the patient both orally and in writing. Monitoring will include prospective review and intervention in:
Therapeutic appropriateness of a patient's drug regimen
Therapeutic duplication in the patient's drug regimen
Appropriateness of the route and method of administration
Degree of patient compliance with the prescribed drug regimen
Drug-drug, drug-food, drug-laboratory, or drug disease interactions
Clinical and pharmacokinetic laboratory data to evaluate the efficacy of drug therapy and to anticipate side effects, toxicity, or adverse effects
Physical signs and clinical symptoms relevant to the patient's drug therapy
Performs continuous evaluation of prescribed medications to assure optimal drug therapy.
Provides medication counseling to patients.
Completes medication histories including patient interviews.
Serves as a drug information resource by providing up-to-date drug information to prescribers, other healthcare professionals, patients, and caregivers.
Evaluates pertinent laboratory data, drug-drug and drug-nutrient interactions, monitors for adverse drug effects, and screens for allergies.
Assesses drug safety and efficacy, including evaluation of physical symptoms.
Reports adverse drug events (ADE), near misses, and medication errors in alignment with the VADERS reporting program.
Participates in the Medical Center Medication Utilization Evaluation (MUE) program.
Work Schedule: Monday - Friday, 8:00am - 4:30pm

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education:
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accreditedCollege or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet- Based Test (TOEFL iBT).
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.
Grandfathering Provision. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations. In addition to the basic requirements, the following criteria must be met when determining the grade of candidates.
GS-13 Clinical Pharmacy Specialist
Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level which includes knowledge of professional pharmacy practice, ability to communicate orally and in writing to both patients and health care staff, knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
Pharmacists assigned to this position must demonstrate the following Knowledge, Skills, and Abilities (KSAs):
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards for their program area.
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: Pain management experience and DEA license preferred.

The full performance level of this vacancy is GS-13. The actual grade at which an applicant may be selected for this vacancy is GS-13.
